| 【】目的 探讨血清铁蛋白(SF)、胰岛素样生长因子结合蛋白3(IGFBP-3)及可溶性肿瘤坏死因子受体P55(sTNFR-P55)与乳腺癌根治术后复发转移的关系。方法 选取2021年4月至2022年4月在我院治疗的乳腺癌患者120例,成功接受乳腺癌根治术,根据随访期间是否发生复发转移,将患者分为复发转移和未复发转移组,比较两组患者临床基线资料及血清SF、IGFBP-3、sTNFR-P55水平等实验室指标差异,同时分析血清SF、IGFBP-3、sTNFR-P55水平与乳腺癌根治术后复发转移的关系,以及复发转移的影响因素。结果 最终有120例患者完成随访,32例发生复发转移,复发转移率为26.67%。复发转移患者肿瘤大小≥2cm、TNM分期Ⅲ、组织学3级比例分别为46.88%、56.25%和53.13%,高于未复发转移患者(P<0.05)。复发转移患者血清SF、IGFBP-3和sTNFR-P55分别为205.50(182.40,295.55)ng/ml、550.45(380.56,750.46)pg/ml和112.50(75.56,150.65)pg/ml,明显高于未复发转移患者(P<0.05)。Cox比例回归分析显示:TNM分期、分化程度、SF、IGFBP-3和sTNFR-P55是乳腺癌根治术后复发转移的影响因素(P<0.05)。血清SF、IGFBP-3和sTNFR-P55预测复发转移的ROC曲线下面积分别为0.886、0.956和0.787,P<0.05。结论 血清SF、IGFBP-3及sTNFR-P55是乳腺癌根治术后复发转移的独立影响因素,三者在预测复发转移方面有一定应用价值,其中IGFBP-3预测价值最好。 |

| Objective: To investigate the relationship between serum ferritin (SF), insulin-like growth factor binding protein 3 (IGFBP-3), soluble tumor necrosis factor receptor P55 (sTNFR-P55) and recurrence and metastasis of breast cancer after radical mastectomy. Methods: 120 patients with breast cancer who were treated in our hospital from April 2021 to April 2022 were selected and successfully underwent radical surgery for breast cancer. According to whether there was recurrence and metastasis during the follow-up period, the patients were divided into recurrence and metastasis group and non recurrence and metastasis group. The clinical baseline data, serum SF, IGFBP-3 and sTNFR-P55 of the two groups were compared, and the influencing factors of recurrence and metastasis were analyzed. Results: Finally, 120 patients completed follow-up, and 32 cases experienced recurrence and metastasis, with a recurrence and metastasis rate of 26.67%. The proportions of tumor size ≥ 2cm, organizational grading 3 grade , and low differentiation in patients with recurrence and metastasis were 46.88%, 56.25%, and 53.13%, respectively, which were higher than those in patients without recurrence and metastasis (P<0.05).The serum levels of SF, IGFBP-3, and sTNFR-P55 in patients with recurrence and metastasis were 205.50 (182.40, 295.55) ng/ml, 550.45 (380.56, 750.46) pg/ml, and 112.50 (75.56,150.65) pg/ml, respectively, which were significantly higher than those in patients without recurrence and metastasis (P<0.05). Cox proportional regression analysis showed that TNM stage, organizational grading, SF, IGFBP-3 and sTNFR-P55 were the influencing factors of recurrence and metastasis of breast cancer after radical surgery (P<0.05). The areas under the ROC curves for predicting recurrence and metastasis using SF, IGFBP-3, and sTNFR-P55 were 0.886, 0.956, and 0.787, respectively, P<0.05. Conclusion: Serum SF, IGFBP-3 and sTNFR-P55 are independent factors influencing the recurrence and metastasis of breast cancer after radical surgery. The three factors have certain application value in predicting the recurrence and metastasis, of which IGFBP-3 has the best predictive value. |
