      目的 探讨慢性自发性荨麻疹患者血清T细胞免疫球蛋白黏蛋白3(Tim3)、核苷酸结合寡聚化结构域样受体蛋白4(NLRC4)表达水平与病情的关系。方法 选取2021年1月至2024年3月延安大学咸阳医院皮肤科收治的107例慢性自发性荨麻疹患者纳入荨麻疹组,按病情分为低活动度组(n=34)、中活动度组(n=46)和高活动度组(n=27)。另选取同期本院107例无过敏/自身免疫病的健康体检者纳入无荨麻疹组。采用酶联免疫吸附试验(ELISA)法检测受试者血清Tim3、NLRC4表达;分析Tim3、NLRC4与病情程度的相关性;分析患者发生高活动度荨麻疹的影响因素;评估Tim3、NLRC4表达水平对高活动度慢性自发性荨麻疹的诊断价值。结果 较无荨麻疹组,荨麻疹组患者血清Tim3水平降低,NLRC4水平升高,差异有统计学意义(P<0.05)。高活动度组患者血清Tim3水平较中、低活动度组患者降低,NLRC4水平较中、低活动组患者升高差异有统计学意义(P<0.05)。荨麻疹组患者血清Tim3表达水平与病情程度呈负相关(rs=-0.531,P<0.05),NLRC4表达水平与病情程度呈正相关(rs=0.526,P<0.05)。Tim3、NLRC4表达水平是患者发生高活动度慢性自发性荨麻疹的影响因素(OR=0.596,95%CI:0.399~0.891;OR=2.324,95%CI:1.335~4.047;P均<0.05)。Tim3、NLRC4联合诊断高活动度慢性自发性荨麻疹患者的曲线下面积优于Tim3(Z=2.429,P<0.05)、NLRC4(Z=2.167,P<0.05)单一诊断。结论 慢性自发性荨麻疹患者血清Tim3表达降低、NLRC4表达升高,二者与患者病情程度存在相关性,二者联合诊断高活动度慢性自发性荨麻疹患者的价值较高。

      Objective To discuss the relationship between the expression of serum T cell immunoglobulin and mucin domain-3(Tim3) and NLR family CARD domain-containing protein 4(NLRC4) in patients with chronic spontaneous urticaria and disease severity. Methods From January 2021 to March 2024, 107 patients with chronic spontaneous urticaria who were admitted to Department of Dermatology, Yan'an University Xianyang Hospital were selected and included in the urticaria group. According to the severity of the condition, they were classified into a low activity group(n=34), a moderate activity group(n=46), and a high activity group(n=27). And 107 healthy individuals without allergies/autoimmune diseases were selected and included in the urticaria group and the non-urticaria group. Enzyme linked immunosorbent assay(ELISA) method was used to detect serum Tim3 and NLRC4 in subjects. The correlation between Tim3, NLRC4 and the severity of the disease was analyzed. The influencing factor of high-activity urticaria in patients was analyzed. The diagnostic value of Tim3 and NLRC4 for high-activity chronic spontaneous urticaria was evaluated. Results The urticaria group had lower serum Tim3 and higher NLRC4 than the non-urticaria group(P<0.05). The high activity group had lower serum Tim3 than the medium and low activity groups, and higher NLRC4 than the medium and low activity groups(P<0.05). The serum Tim3 in the urticaria group was negatively correlated with disease severity(rs=-0.531,P<0.05), while NLRC4 was positively correlated with disease severity(rs=0.526,P<0.05). The levels of Tim3 and NLRC4 were influencing factors for the occurrence of high-activity chronic spontaneous urticaria in patients [OR=0.596(95%CI:0.399~0.891), OR=2.324(95%CI:1.335~4.047), P<0.05]. The area under the curve for the combination diagnosis of Tim3 and NLRC4 in high-activity chronic spontaneous urticaria patients was superior to single diagnosis of Tim3(Z=2.429, P<0.05) and NLRC4(Z=2.167, P<0.05). Conclusion Serum Tim3 decreases and NLRC4 increases in patients with chronic spontaneous urticaria. There is a correlation between the two and the disease severity. In addition, the combination of the two has a high diagnostic value in patients with high activity chronic spontaneous urticaria.
