| Objective To investigate the relationship between triglyceride glucose(TyG) index and proteinuria in elderly patients with metabolic syndrome(MS) in the community. Method A retrospective study was conducted on 200 MS patients who participated in elderly physical examinations in our hospital from January to December 2024 as the observation group, and 200 healthy elderly people who participated in physical examinations during the same period were selected as the control group. Indicators such as blood lipids, blood glucose, and renal function were measured in patients and healthy individuals undergoing physical examinations, and the TyG index was calculated. The TyG index and UACR were compared between the observation group and the control group. According to the TyG trilemma, the observation group was divided into the low TyG(L-TyG) group(n=67), medium TyG(M-TyG) group(n=66), and high TyG(H-TyG) group(n=67) to evaluate the relationship between TyG index and clinical characteristics of MS patients; laboratory indicators of three groups of patients were compared, and Spearman correlation analysis was used to investigate the relationship between TyG index and laboratory indicators. According to whether proteinuria occurred in the observation group, they were divided into the proteinuria group(n=80) and simple MS group(n=120). The baseline data and laboratory indicators of the two groups of patients were compared, and logistic regression was used to analyze the influencing factors of proteinuria in MS patients; the efficacy of various indicators for predicting the risk of proteinuria in MS patients was assessed using receiver operating characteristic(ROC) curves. Result The TyG index and UACR of the observation group were higher than those of the control group(P<0.05). Compared with the L-TyG group and M-TyG group, the FPG, HbA1c, TC, TG, LDL-C, UA, and UACR of the H-TyG group significantly increased, while HDL-C significantly decreased(P<0.05). Relevant analysis showed that TyG index was positively correlated with FPG, HbA1c, TC, TG, LDL-C, UA, UACR, and negatively correlated with HDL-C(P<0.05). The levels of TyG, TC, UA, BUN, and UACR in the proteinuria group were higher than those in the simple MS group, while HDL-C was lower than that in the simple MS group(P<0.05). Logistic regression showed that TyG index, TC, HDL-C, and UACR were independent influencing factors for proteinuria in MS patients(P<0.05). ROC curve analysis showed that the UACR+HDL-C+TC+TyG index predicted the area under the proteinuria curve(AUC) of MS patients to be 0.819, with sensitivity and specificity of 80.27% and 82.43%, respectively, which were significantly higher than TC, HDL-C, UACR, and TyG(P<0.05). Conclusion There is a close relationship between TyG index and the occurrence and development of MS. It has a high predictive power for proteinuria in MS patients and can be used as an important indicator for evaluating the risk of renal damage in MS patients. |
