      目的 探究硫酸沙丁胺醇联合丙酸氟替卡松治疗小儿支气管哮喘(PBA)的临床价值。方法 对郑州大学第三附属医院2020年1月至2021年2月收治的120例PBA患儿临床资料开展回顾性研究,依照临床采用的干预手段分组:试验组纳入61例患儿,对照组收录59例患儿。对照组采用丙酸氟替卡松雾化吸入治疗,试验组在对照组基础上加硫酸沙丁胺醇雾化吸入。于治疗前后检测两组患者肺功能、炎症因子,统计两组患儿症状改善情况、住院时间、呼出气一氧化氮及治疗期间用药安全性。结果 治疗后,试验组和对照组的肺功能指标呼气峰值流量(PEF)、第1秒用力呼气容积(FEV1)、用力肺活量(FVC)均较治疗前改善(P<0.05),试验组各指标的改善幅度均优于对照组(P<0.05)。治疗后,试验组和对照组的哮喘控制测试量表(C-ACT)、呼出气一氧化氮(FeNO)均较治疗前改善(P<0.05),试验组各指标的改善幅度均优于对照组(P<0.05)。治疗后,试验组和对照组的肿瘤坏死因子-α(TNF-α)、干扰素γ(IFN-γ)、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)均较治疗前改善(P<0.05),试验组各指标的改善幅度均优于对照组(P<0.05)。试验组患儿喘憋、咳嗽、哮鸣音3类临床表现消退用时及整体住院周期与对照组比较均明显缩短(t=8.858、7.032、6.728、5.770,P均<0.001)。两组不良反应发生率差异无统计学意义(χ2=0.113,P=0.737)。结论 对于PBA,硫酸沙丁胺醇联合丙酸氟替卡松的应用效果较好,可改善患儿炎性反应、肺功能,促进患儿症状消失,且可缩短住院时间,用药安全可靠。

      Objective To explore the clinical value of salbutamol sulfate combined with fluticasone propionate in the treatment of pediatric bronchial asthma(PBA). Methods A retrospective study was conducted on the clinical records of 120 children with PBA admitted to the Third Affiliated Hospital of Zhengzhou University from January 2020 to February 2021. Based on the different clinical interventions, the patients were divided into an experimental group of 61 cases and a control group of 59 cases. The control group was treated with nebulized inhalation of fluticasone propionate, while the experimental group received nebulized inhalation of salbutamol sulfate in addition to the treatment of control group. The lung function and inflammatory factors of the two groups were assessed before and after treatment, and the improvement of symptoms, length of hospital stay, fractional exhaled nitric oxide(FeNO), and medication safety during treatment were also recorded. Results After treatment, the pulmonary function indicators, namely peak expiratory flow(PEF), forced expiratory volume in the first second(FEV1), and forced vital capacity(FVC), showed improvements in both the experimental and control groups compared to the pre-treatment levels(P<0.05). The degree of improvement for each indicator in the experimental group was significantly greater than that in the control group(P<0.05). After the treatment, the Childhood Asthma Control Test(C-ACT) scores and FeNO levels in both groups improved compared to pre-treatment values(P<0.05), with the experimental group showing significantly greater improvement(P<0.05). Similarly, after treatment, the serum levels of tumor necrosis factor alpha(TNF-α), interferon gamma(IFN-γ), interleukin-4(IL-4), and interleukin-10(IL-10) in both groups improved compared to before treatment(P<0.05), and the improvements in the experimental group were superior to the control group(P<0.05). The time for the resolution of clinical symptoms such as wheezing, coughing, and whistling rales, as well as the overall length of hospitalization, in the experimental group were notably shorter than those in the control group(t= 8.858, 7.032, 6.728, 5.770, all P<0.001). There was no statistically significant disparity in the incidence of adverse reactions between the two groups(χ2=0.113, P=0.737). Conclusion For PBA, the combination of salbutamol sulfate and fluticasone propionate is highly effective, which can alleviate the inflammatory reactions, improve lung function, accelerate the resolution of symptoms, and shorten the length of hospitalization, ensuring medication safety and reliability.
