| 目的 探讨血清微小RNA-124-3p(miR-124-3p)、微小RNA-126-5p(miR-126-5p)、微小RNA-199a-3p(miR-199a-3p)表达与毛细支气管炎患儿病情严重程度、肺功能的关系。方法 收集2021年8月至2023年8月张家口市妇幼保健院收治的首次确诊的124例毛细支气管炎患儿的临床资料(观察组)。根据不同病情严重程度将患儿分为轻中症组(n=89)和重症组(n=35)。另选同期张家口市妇幼保健院体检的125例健康幼儿作为对照组。采用实时荧光定量聚合酶链式反应(PCR)检测血清miR-124-3p、miR-126-5p、miR-199a-3p表达水平。采用Pearson法分析毛细支气管炎患儿血清miR-124-3p、miR-126-5p、miR-199a-3p水平与肺功能指标相关性。受试者工作特征(ROC)曲线分析血清miR-124-3p、miR-126-5p、miR-199a-3p水平对重症毛细支气管炎患儿的预测价值。结果 与对照组相比,观察组血清miR-124-3p、miR-199a-3p水平及潮气量(VT)、达峰时间比(TPTEF/TE)、达峰容积比(VPEF/VE)降低,呼吸频率(RR)、miR-126-5p水平升高(P<0.05)。重症组RR、血清miR-126-5p水平高于轻中症组,TPTEF/TE、VPEF/VE及血清miR-124-3p、miR-199a-3p水平低于轻中症组(P<0.05)。毛细支气管炎患儿肺功能指标TPTEF/TE、VPEF/VE与血清miR-124-3p、miR-199a-3p水平呈正相关,与miR-126-5p呈负相关(P<0.05)。血清miR-124-3p、miR-126-5p、miR-199a-3p联合预测重症毛细支气管炎患儿的曲线下面积(AUC)为0.963,预测效能优于3者单一检测(AUC=0.816、0.834、0.831,Z=3.826、3.264、3.456,P<0.05)。结论 毛细支气管炎患儿血清中miR-124-3p、miR-199a-3p水平下调,miR-126-5p水平上调,其水平变化与患儿病情严重程度、肺功能下降有关,3者联合检测或可作为辅助预测毛细支气管炎患儿病情指标。 |

| Objective To investigate the relationship between serum microRNA-124-3p(miR-124-3p), microRNA-126-5p(miR-126-5p), and microRNA-199a-3p(miR-199a-3p) expression levels and the severity of disease and lung function in children with bronchiolitis. Methods A prospective study was conducted, collecting clinical data from 124 paediatric patients with newly diagnosed bronchiolitis admitted to Zhangjiakou Maternal and Child Health Hospital between August 2021 and August 2023(observation group). Patients were categorised into a mild-moderate group(n=89) and a severe group(n=35) based on disease severity. Another 125 healthy toddlers who underwent physical examination in the same hospital during the same period were selected as the control group. The expression levels of serum miR-124-3p, miR-126-5p, and miR-199a-3p were detected using real-time fluorescent quantitative polymerase chain reaction(RT-qPCR). The Pearson method was applied to analyze the correlation between serum miR-124-3p, miR-126-5p, miR-199a-3p levels and lung function indicators in children with bronchiolitis. Receiver operating characteristic(ROC) curve analysis was used to evaluate the predictive value of serum miR-124-3p, miR-126-5p, and miR-199a-3p levels for severe bronchiolitis in paediatric patients. Results The expression levels of miR-124-3p and miR-199a-3p in the serum of the observation group, as well as tidal volume(VT), ratio of time to peak tidal expiratory flow to total expiratory time(TPTEF/TE), and ratio of volume to peak expiratory flow to total exhaled volume(VPEF/VE), were significantly lower than those in the control group, while the respiratory rate(RR) and the expression level of miR-126-5p were significantly higher(P<0.05). The RR and the level of serum miR-126-5p in the severe group were higher than those in the mild-moderate group, while the TPTEF/TE, VPEF/VE, and levels of serum miR-124-3p and miR-199a-3p were lower than those in the mild-moderate group(P<0.05). Pearson correlation analysis showed that lung function indicators(TPTEF/TE, VPEF/VE) were positively correlated with serum miR-124-3p and miR-199a-3p levels, and negatively correlated with miR-126-5p levels in children with bronchiolitis(P<0.05). The combined detection of serum miR-124-3p, miR-126-5p, and miR-199a-3p panel demonstrated an area under the curve(AUC) of 0.963 for predicting severe bronchiolitis in paediatric patients, exhibiting superior predictive efficacy compared to individual assays(0.816, 0.834, and 0.831, respectively)(Z=3.826, 3.264, and 3.456, P<0.05). Conclusion The levels of miR-124-3p and miR-199a-3p are down-regulated and miR-126-5p is up-regulated in the serum of children with bronchiolitis. These changes in expression levels are related to the severity of the disease and the decline in lung function. The combination of these three markers may serve as a supplementary predictor for assessing disease severity in children with bronchiolitis. |
