目的：研究瑞士医迈斯(EMS)系统与牙周翻瓣术式对中重度慢性牙周炎患者的疗效及美容效果的影响。方法：选择2017年3月到2018年3月在我院接受治疗的中重度型慢性牙周炎患者100例为研究对象。采用随机数字表法将患者分为观察组及对照组各50例,观察组采用EMS系统治疗,对照组采用牙周翻瓣术治疗,治疗后12个月随访。比较两组患者治疗12个月后的牙周探诊深度(PD)和牙龈退缩(GR),临床附着丧失(CAL),以及探诊出血(BOP),同时比较两组患者的Buser红色美学指数(PES)美容效果评分。结果：对于单根牙而言,治疗后观察组的GR及CAL水平较对照组明显更低(P<0.05),但两组治疗后的PD水平及BOP阳性率相比,差异不显著(P>0.05)。对于多根牙而言,治疗后观察组的PD、CAL水平及BOP阳性率较对照组明显更高,GR水平较对照组明显更低(P<0.05)。治疗后两组的PES美容效果评分较治疗前均明显更高(P<0.05)。结论：对于有中重度慢性牙周炎的患者,存在单根牙情况时宜选择EMS系统治疗,而存在多根牙情况时则建议使用牙周翻瓣术进行治疗,两种术式的美容效果基本相当,值得临床关注与推广。

Objective: To study the effect of Electro Medical Systems(EMS)system and periodontal flap operation on moderate and severe chronic periodontitis patients. Methods: 100 patients with moderate and severe chronic periodontitis who were treated in our hospital from March 2017 to March 2018 were studied. The patients were divided into observation group (EMS system treatment) and control group (periodontal valvus treatment) with 50 cases in each group. Follow up for 12 months, the periodontal probing depth (PD) and gingival recession (GR), clinical attachment loss (CAL), and bleeding on probing (BOP) . Before treatment and 12 months after treatment were compared between the two groups. The Buser red aesthetic index (PES) beauty score before treatment and 12 months after treatment were compared between the two groups. Results: There was no significant difference in PD,GR,CAL level and BOP positive rate between the two groups before treatment for a single tooth (P>0.05). The levels of GR and CAL in the observation group were significantly lower than the control group (P<0.05), but there was no significant difference in the level of PD and the positive rate of BOP between the two groups after treatment (P>0.05). For multiple teeth, there was no significant difference in PD,GR,CAL level and BOP positive rate between the two groups before treatment (P>0.05). The level of PD,CAL and the positive rate of BOP in the observation group were significantly higher than the control group, and the level of GR in the observation group was significantly lower than the control group (P<0.05). There was no significant difference in PES beauty score between the two groups before and after treatment (P>0.05). After treatment, the PES score of the two groups was significantly higher than before treatment (P<0.05). Conclusion: For patients with moderate and severe chronic periodontitis, EMS system should be chosen for treatment of single tooth, and periodontal flap should be used for the treatment of multiple teeth. The cosmetic effects of the two methods are basically the same. It is worthy of clinical attention and promotion.
