      目的 探讨海拔高度对慢性高原病(CMS)患者睡眠及病情的影响。方法 武警青海总队医院2013年5~8月对海拔3780 m地区长期居住的10名CMS患者进行多导睡眠监测,患者移居到海拔2260 m 2周后再重复睡眠监测。结果 CMS患者在3780 m地区主要以浅睡眠为主,2260 m地区以深睡眠为主。海拔3780 m呼吸暂停低通气指数显著高于海拔2260 m,差异有统计学意义(P<0.05);海拔3780 m平均动脉血氧饱和度(SaO2)、最低SaO2显著低于海拔2260 m,差异有统计学意义(P<0.05);从海拔3780 m移到2260 m地区2周后,CMS评分、血红蛋白均显著降低,差异有统计学意义(P<0.05)。结论 高原地区睡眠紊乱引起的夜间低氧血症是CMS发病和发展的重要因素。
英文摘要:
      Objective To study the effect of altitude on the status of sleep and conditions of chronic mountain sickness (CMS).Methods We measured the sleep structure and quality in 10 CMS patients at altitude 3 780 m and after CMS patients transferred to 2 260 m for two weeks during May of 2013 to August by polysomnography.Results Light sleep was the most prevalent type of sleep structure in CMS patients at 3 780 m while deep sleep was the most prevalent type at 2 260 m. The apnea/hypopnea indices were significantly higher in CMS patients at 3 780 m than at 2 260 m.The mean nocturnal arterial oxygen saturation (SaO2) and the lowest SaO2 were significantly lower at 3 780 m than at 2 260 m. The CMS score and hemoglobin concentration were significantly decreased when CMS patients were transferred to 2 260 m for two weeks.Conclusion Thenocturnal oxygen desaturation due to sleep disorder plays an important role in the occurrence and development of CMS.
