      目的 观察血清及肾组织中Toll样受体4(TLR4)、肝细胞生长因子(HGF)水平与急性肾损伤病变程度的关系。方法 上海市松江区中心医院肾内科2015年5月至2017年5月收治的90例急性肾损伤患者根据疾病分期不同,分为A组33例(急性肾损伤Ⅰ期)、B组29例(急性肾损伤Ⅱ期)、C组28例(急性肾损伤Ⅲ期)。分别取患者静脉血及穿刺活检肾组织,检测血清及肾组织中TLR4、HGF的表达情况,并以急性生理与慢性健康状况评分系统Ⅲ(APACHE Ⅲ)分析其与急性肾损伤病变程度的相关性。结果 C组血清TLR4、HGF水平高于B组、A组,B组TLR4、HGF水平高于A组,差异均有统计学意义(P<0.05)。3组肾组织中TLR4、HGF阳性表达率差异有统计学意义(P<0.05);B组、C组TLR4、HGF阳性表达率高于A组,C组TLR4、HGF阳性表达率高于B组,差异均有统计学意义(P<0.017)。TLR4、HGF水平随肾损伤病变程度的加重而升高,TLR4与APACHE Ⅲ评分呈正相关(r=0.942,P<0.05),HGF与APACHE Ⅲ评分呈正相关(r=0.926,P<0.05)。结论 不同分期急性肾损伤患者血清及肾组织中TLR4、HGF水平存在差异。TLR4、HGF与急性肾损伤病变程度呈正相关。

      Objective To observe the relationship between the levels of Toll-like receptor 4 (TLR4) and hepatocyte growth factor (HGF) in serum and renal tissues and the severity of acute kidney injury. Methods Ninety patients with acute kidney injury admitted to the Department of Nephrology, Songjiang District Central Hospital, Shanghai from May 2015 to May 2017 were divided into 33 patients in group A (phase I acute kidney injury), 29 patients in group B (phase Ⅱ acute kidney injury), and 28 cases in group C (phase Ⅲ acute kidney injury). The venous blood and biopsy kidney tissues were taken respectively for the detection of the expression of TLR4 and HGF in serum and renal tissues, and their correlation with the severity of acute kidney injury was analyzed using acute physiology and chronic health evaluation scoring system Ⅲ (APACHE Ⅲ). Results The levels of serum TLR4 and HGF in group C were higher than those in group B and group A. The levels of TLR4 and HGF in group B were higher than those in group A (P<0.05). The positive expression rates of TLR4 and HGF in three groups of renal tissues were statistically significant (P<0.05). The positive expression rates of TLR4 and HGF in group B and group C were higher than those in group A. The positive expression rates of TLR4 and HGF in group C were higher than those in group B (P<0.017). The levels of TLR4 and HGF increased with the severity of renal injury, and the levels of TLR4 were significantly correlated with the scores of acute physiology and chronic health evaluation scoring system Ⅲ (r=0.942, P<0.05). HGF was positively correlated with APACHE Ⅲ score (r=0.926, P<0.05). Conclusions The levels of TLR4 and HGF in serum and renal tissues of patients with different stages of acute kidney injury are different. TLR4 and HGF are positively correlated with the degree of acute kidney injury.
