      目的 比较肌萎缩侧索硬化症(ALS)患者斜方肌与胸锁乳突肌、腹直肌与脊旁肌的肌电图差异,探讨斜方肌及腹直肌肌电图的作用及意义,为ALS早期诊断提供参考。方法 回顾性分析2014年4月至2018年8月安徽中医药大学神经病学研究所附属医院确诊的43例ALS患者的肌电图资料,其中在延髓段成功获取39例患者斜方肌及42例患者胸锁乳突肌肌电图资料,在胸髓段成功获取37例患者腹直肌及10例患者胸段脊旁肌肌电图资料,均包括肌肉在安静状态下的自发电位、在小收缩时的运动单位动作电位(MUAP)及大力收缩时的募集相。比较延髓段斜方肌和胸锁乳突肌,胸髓段腹直肌和胸段脊旁肌的自发电位、MUAP、募集相的阳性率。结果 39例斜方肌肌电图的自发电位(87.2%)、MUAP(92.3%)、募集相(84.6%)均高于42例胸锁乳突肌的自发电位(35.7%)、MUAP(59.5%)、募集相(45.2%),差异均有统计学意义(P<0.05)。37例腹直肌肌电图的自发电位(62.2%)、MUAP(64.8%)、募集相(75.6%)均高于10例胸段脊旁肌的自发电位(20.0%)、MUAP(20.0%)、募集相(50.0%),两者自发电位和MUAP差异有统计学意义(P<0.05),但募集相差异无统计学意义(P>0.05)。结论 对于ALS患者,斜方肌较胸锁乳突肌、腹直肌较胸段脊旁肌更易出现肌电异常,斜方肌及腹直肌肌电图在ALS诊断中有重要意义,对疑诊ALS的患者应重视斜方肌及腹直肌肌电图的检查。

      Objective To compare the differents of EMG Results between trapezius and sternocleidomastoid muscle, rectus abdominis and paravertebral muscles of amyotrophic lateral sclerosis Patients (ALS) and discusse the role and significance of trapezius and rectus abdominis in EMG, for providing reference for early diagnosis of ALS. Methods We retrospectively analyzed the EMG data of the 43 patients diagnosed by the affiliated hospital of the Neurology Institute of Anhui University of Chinese Medicine from April 2014 to August 2018.The spontaneous potential of the muscle in a quiet state, motor unit action potential (MUAP) of the muscle in mild contraction and recruitment phase in heavy contraction were got throught EMG implemented in the medulla oblongata innervation muscles (trapezius of 39 patients, sternocleidomastoid of 42 patients) and thoracic cord innervation muscles (rectus abdominis of 37 patients and the paravertebral muscles of 10 patients).The positive rates of spontaneous potential, MUAP and recruitment phase were compared between the trapezius and the sternocleidomastoid muscle, the rectus abdominis and the paravertebral muscles.Results The abnormal percentage of trapezius EMG in spontaneous potential (87.2%), MUAP (92.3%) and recruitment phase (84.6%) were significant higher than sternocleidomastoid EMG in spontaneous potential (35.7%), MUAP (59.5%) and recruitment phase (45.2%) (P<0.05). And abnormal percentage of rectus abdominis EMG in spontaneous potential (62.2%), MUAP (64.8%) and recruitment phase (75.6%) were higher than paravertebral muscles EMG in spontaneous potential (20.0%), MUAP (20.0%) and recruitment phase (50.0%) too, the first two EMG items of the two group muscles were significantly different (P<0.05), while the last item was no (P>0.05).Conclusion In patients with ALS, the trapezius and rectus abdominis are more prone to abnormal in EMG than the sternocleidomastoid and the paravertebral muscles. The trapezius and rectus abdominis EMG have great significance in the diagnosis of ALS, we should pay more attention to them for possible ALS patients.
