      目的 探讨肺腺癌患者增殖细胞核抗原(Ki-67)表达与表皮生长因子受体(EGFR)突变的关系及Ki-67表达对患者预后的影响。方法 收集2016年1~12月在安徽省胸科医院住院并行CT引导下经皮肺穿刺活检确诊为ⅢB期~Ⅳ期的肺腺癌患者64例。使用免疫组织化学方法检测患者病理组织中Ki-67的表达情况,以Ki-67≥26%定义为高表达,将患者分为Ki-67高表达组(40例)和低表达组(24例),比较两组一般资料、EGFR突变情况及总生存率和无进展生存率的差异。结果 年龄<75岁的Ki-67高表达患者占比高于年龄>75岁者,差异有统计学意义(P<0.05)(χ2=4.022,P=0.045)。Ki-67高表达组EGFR突变阳性率高于Ki-67低表达组,差异有统计学意义(P<0.05)(χ2=4.104,P=0.043)。Kaplan-Meier生存分析显示,Ki-67高表达组总生存率低于Ki-67低表达组,差异有统计学意义(P<0.05)(χ2=7.457,P=0.006),两组无进展生存率比较,差异无统计学意义(χ2=2.563,P=0.109)。结论 Ki-67高表达者EGFR突变阳性率较Ki-67低表达者高,总生存率较Ki-67低表达者低,可能是肺腺癌患者的潜在预后指标。

      Objective To investigate the relationship between the expression of proliferating cell nuclear antigen (Ki-67) and epidermal growth factor receptor (EGFR) mutation in patients with lung adenocarcinoma, and the effect of Ki-67 expression on the prognosis of patients. Methods From January to December 2016, 64 patients with stage ⅢB-Ⅳ lung adenocarcinoma diagnosed by CT guided percutaneous lung biopsy in Anhui Chest Hospital were collected. Immunohistochemical method was used to detect the expression of Ki-67 in the pathological tissues of the patients. Ki-67 ≥ 26% was defined as high expression. The patients were divided into Ki-67 high expression group (40 cases) and low expression group (24 cases). The general data, EGFR mutation, overall survival rate and progression free survival rate of the two groups were compared. Results The proportion of Ki-67 high expression patients aged<75 years old was higher than that of those aged > 75 years old(χ2=4.022, P=0.045).The positive rate of EGFR mutation in Ki-67 high expression group was significantly higher than that in Ki-67 low expression group(χ2=4.104, P=0.043). Kaplan Meier survival analysis showed that the overall survival rate of high Ki-67 expression group was significantly lower than that of low Ki-67 expression group(χ2=7.457, P=0.006). There was no significant difference in progression free survival between the two groups (χ2=2.563, P=0.109). Conclusions The positive rate of EGFR mutation in patients with high Ki-67 expression is higher than that in patients with low Ki-67 expression, and the overall survival rate is lower than that in patients with low Ki-67 expression, which may be a potential prognostic indicator of lung adenocarcinoma.
