Objective To explore the value of fetal umbilical artery and middle cerebral artery hemodynamics indexesin predicting fetal distress.Methods Seventy six pregnant women with fetal distress who were admitted to the Xuancheng People's Hospital from June 2018 to June 2020 were selected as the observation group, and 68 pregnant women with healthy fetuses during the same period were selected as the control group. The blood flow resistance index (RI), pulsatility index (PI), peak systolic flow velocity and end-diastolic blood flow velocity ratio (S/D), and RI ratio between MCA and UA (RIMCA/UA), PI ratio between MCA and UA (PIMCA/UA), and S/D ratio between MCA and UA (S/DMCA/UA) at 32 to 41 weeks of gestation were compared, the receiver operating curve (ROC) was used to analyze the value of RIMCA/UA, PIMCA/UAand S/DMCA/UAin predicting fetal distress.Results RI(0.86±0.13), PI(1.05±0.14), S/D(2.59±0.34) of UA in the observation group were higher than those in the control group[RI(0.59±0.08), PI(0.88±0.11), S/D(2.03±0.31)](RI:t=14.799, P<0.001, PI:t=8.036, P<0.001, S/D:t=10.285, P<0.001). The RI(0.62±0.09), PI(1.19±0.12), S/D(3.85±0.53) of MCA in the observation group were lower than those in the control group[RI(0.80±0.13), PI(1.47±0.18), S/D(4.76±0.64)](RI:t=9.742, P<0.001, PI:t=11.086, P<0.001, S/D:t=9.327, P<0.001). The RIMCA/UA(0.69±0.11), PIMCA/UA(1.16±0.13), S/DMCA/UA(1.45±0.17) in the observation group were lower than those in the control group[RIMCA/UA(1.15±0.22), PIMCA/UA(1.62±0.21), S/DMCA/UA(2.27±0.25)](RIMCA/UA:t=16.119, P<0.001, PIMCA/UA:t=15.981, P<0.001, S/DMCA/UA:t=23.221, P<0.001).The area under curve (AUC) of RIMCA/UA, PIMCA/UAand S/DMCA/UA to assess fetal distress was 0.937, 0.841, 0.846, respectively.Conclusions The hemodynamic parameters of UA and MCA of the fetus with intrauterine distress have abnormal changes, and RIMCA/UA, PIMCA/UAand S/DMCA/UA have important value in theprediction of fetal distress. |
