      目的 探讨结直肠癌患者18F-FDG PET/CT代谢参数与临床特征及癌胚抗原(CEA)、外周血中性粒细胞/淋巴细胞比值(NLR)的相关性。方法 回顾性分析2015年1月至2021年12月35例(41个病灶)在安徽医科大学第一附属医院行18F-FDG PET-CT显像并经病理学确诊为结直肠癌或高级别上皮内瘤变患者的临床资料,获取其肿瘤最大标准化摄取值(SUVmax)、平均标准化摄取值(SUVmean)、肿瘤代谢体积(MTV)、病灶糖酵解总量(TLG)等PET/CT代谢参数,采用Spearman相关分析上述PET/CT代谢参数与临床特征(患者性别、年龄、肿瘤部位、肿瘤最大径、分化程度)、肿瘤标志物(血清CEA)及临床预后指标(NLR)的相关性。结果 原发灶PET/CT代谢参数SUVmax、SUVmean、MTV、TLG与患者性别的相关系数r分别为-0.045、0.056、0.235及0.115(P均>0.05),与患者年龄的相关系数r分别为-0.131、-0.156、-0.063及-0.231(P均>0.05),与肿瘤部位的相关系数r分别为0.435、0.431、0.432及0.554(P均<0.05)。MTV、TLG与肿瘤最大径的相关系数r分别为0.632(P<0.05)、0.633(P<0.05)。SUVmax、MTV及TLG与肿瘤分化程度的相关系数r分别为-0.57、-0.641及-0.713(P均<0.05)。TLG与血清CEA的相关系数r=0.625(P<0.05),MTV、TLG与NLR的相关系数r分别为0.561(P<0.05)、0.672(P<0.05)。结论 结直肠癌患者原发灶各代谢参数均与肿瘤部位相关,其中MTV、TLG与肿瘤最大径均呈正相关性,SUVmax、MTV及TLG与肿瘤分化程度均呈负相关,TLG与肿瘤标志物血清CEA呈正相关性,TLG、MTV与临床预后指标NLR呈正相关。

      Objective To investigate the correlation between metabolic parameters of 18F-FDG PET/CT imaging and clinical features, carcinoembryonic antigen(CEA) and peripheral blood neutrophil/lymphocyte ratio(NLR) of colorectal cancer. Methods The clinical data of 35patients (41lesions) who received 18F-FDG PET-CT imaging and were pathologically confirmed as colorectal cancer or high-grade intraepithelial neoplasia in the First Affiliated Hospital of Anhui Medical University from January 2015 to December 2021 were retrospectively analyzed. The metabolic parameters of maximum standardized uptake value (SUVmax), mean standardized uptake value (SUVmean), metabolic tumor volume (MTV) and total lesion glycolysis (TLG) values were measured. Spearman correlation analysis was used to investigate the correlation between PET/CT metabolic parameters and clinical characteristics (gender, age, tumor site, tumor maximum diameter, degree of differentiation), tumor markers (CEA) and clinical prognostic indicators (NLR).Results The correlation coefficients of primary PET/CT metabolic parametersSUVmax, SUVmean, MTV, TLG and gender were-0.045, 0.056, 0.235, 0.115(all P>0.05), respectively, and age were -0.131,-0.156, -0.063, -0.231(all P>0.05), and tumor location were0.435, 0.431, 0.432, 0.554(all P<0.05). The correlation coefficient of MTV, TLG and tumor maximum diameter was 0.632 (P<0.05), 0.633 (P<0.05), respectively. The correlation coefficients of SUVmax, MTV and TLG with tumor differentiation degree were-0.57, -0.641, -0.713respectively(all P<0.05). The correlation coefficient between TLG and tumor marker CEA was 0.625 (P<0.05).The correlation coefficient between MTV, TLG and NLR was 0.561(P<0.05),0.672(P<0.05). Conclusions All metabolic parameters of primary CRC are correlated with tumor location.MTV and TLG arepositivelycorrelated with maximum tumor diameter.SUVmax, MTV and TLG arenegativelycorrelated with tumor differentiation degree. TLG ispositivelycorrelated with tumor marker serum CEA. TLG and MTV arepositivelycorrelated with clinical prognosis index.
