      目的 量化评估肝豆状核变性伴冻结步态患者的步态特征和平衡功能。方法 选取2021年2月至2022年2月安徽中医药大学神经病学研究所附属医院收治的30例肝豆状核变性(WD)患者,根据是否冻结步态(FOG)将患者分为WD伴FOG组(n=15例)和WD不伴FOG组(n=15例),并纳入同期健康人为正常对照组(n=15例)。使用基于柔性力敏传感技术和三维测力平台的U型电子步道系统采集提取3组研究对象的步态和平衡参数。应用统一肝豆状核变性评分量表第一部分神经系统评分(UWDRS-I)和改良巴士指数评定表(MBI)对WD患者的神经系统和日常活动能力进行评分。同时采用Spearman相关性对WD伴FOG组的量表评分、平衡参数和步态参数进行相关分析。结果 ①WD伴FOG组双侧的步长、步幅、步速低于WD不伴FOG组和正常对照组,差异有统计学意义(P<0.05),左右侧步态参数对称比差异无统计学意义(P>0.05)。②睁眼站立时,WD伴FOG组压力中心(COP)动摇速度、COP平均动摇角速度和COP内外方向动摇角速度高于WD不伴FOG组和正常对照组,差异有统计学意义(P<0.05);WD伴FOG组COP轨迹长、COP前后方向动摇角速度高于对照组,差异有统计学意义(P<0.05)。③WD伴FOG组患者的步态参数,平衡参数、MBI评分和UWDRS-I评分之间均无相关性(P>0.05)。结论 伴FOG的WD患者存在步态异常和平衡障碍。但伴FOG的WD患者步态、平衡、神经系统功能和日常活动能力之间均无相关关系。
英文摘要:
      Objective Quantify analysis the gait characteristics and balance function in patients with freezing of gait in Wilson,s Disease. Methods A total of 30 patients with WD, admitted to our hospital from February 2021 to February 2022, were enrolled in our study. The participants were divided into freezing of gait group (WD+FOG) (n=15) and non-freezing of gait group (WD-FOG) (n=15) based on the FOG status. And healthy people in the same period (n=15) were included as normal control group. The gait parameters and balance parameters of the subjects were obtained by the U-shape electronic walkway based on the flexible force-sensitive technology and three-dimensional force-measuring platform. The neurological and daily activities of the patients were scored using nervous system component of the Unified Wilson’s Disease Rating Scale (UWDRS-I) and modified barthel index (MBI). And Spearman correlation analysis was used tocarry out correlation analysis on the scale scores, balance parameters and gait parameters of the WD+FOG group. Results ①The step length, stride length and velocity were lower in WD+FOG group than those in WD-FOG group and the control group (P<0.05), and there was no significant difference in the symmetry ratio of left and right gait parameters (P>0.05). ②Under double-limb stance eyes open condition, the center of pressure (COP) trunk sway speed, average away angular velocity and away angular velocity in the medial-lateral direction of the COP were higher in WD+FOG group than those in WD-FOG group and the control group (P<0.05), The total trajectory length and away angular velocity in the medial-lateral direction of the COP were higher in WD+FOG group than the control group (P<0.05). ③There was no correlation is observed between gait parameters, balance parameters, UWDRS-I scores and MBI scores in WD+FOG group.Conclusion Gait disorder and Balance disorder were observed in the patient with FOG in WD. WD patients with FOG identified no associations emerged between walking, balance, nervous system function and abilities of daily living.
