      目的 探究肥胖2型糖尿病(T2DM)患者血浆外泌体微小RNA-155(miR-155)表达水平与内脏脂肪面积(VFA)、胰岛素抵抗(IR)的关系。方法 选取洪湖市人民医院2019年10月至2021年6月新诊断的T2DM患者144例为研究对象,按身体质量指数(BMI)将其分为正常体重组(34例,BMI<24 kg/m2)、超重组(38例,24 kg/m2≤BMI<28 kg/m2)、肥胖组(72例,BMI≥28 kg/m2)。比较各组患者的血浆外泌体miR-155表达水平以及VFA、稳态模型胰岛素抵抗指数(HOMA-IR);Pearson法分析肥胖T2DM患[者血浆外泌体miR-155表达水平、VFA、HOMA-IR与临床指标,及血浆外泌体miR-155表达水平与VFA、HOMA-IR的相关性;多元线性回归分析血浆外泌体miR-155的影响因素。结果 肥胖组VFA、HOMA-IR高于正常体重组、超重组,差异有统计学意义(P<0.05),血浆外泌体miR-155表达水平低于正常体重组、超重组,差异有统计学意义(P<0.05);超重组VFA、HOMA-IR高于正常体重组,血浆外泌体miR-155表达水平低于正常体重组,差异均有统计学意义(P<0.05);肥胖T2DM患者BMI、FINS、腰臀比(WHR)与血浆外泌体miR-155表达水平呈负相关(r=-0.393、-0.406、-0.445,P<0.05),与VFA、HOMA-IR呈正相关(r=0.456、0.362、0.401;r=0.322、0.387、0.350;P均<0.05);肥胖T2DM患者血浆外泌体miR-155表达水平与VFA、HOMA-IR呈负相关(r=-0.588、-0.562,P<0.05);多元线性回归分析显示,VFA、HOMA-IR是miR-155的影响因素(P<0.05)。结论 肥胖T2DM患者血浆外泌体miR-155表达水平较低,其与VFA、IR密切相关,miR-155可能是防治肥胖T2DM的潜在靶标。

      Objective To explore the relationship between plasma exosomal microRNA-155 (miR-155) expression level and visceral fat area (VFA) and insulin resistance (IR) in obese type 2 diabetes mellitus (T2DM) patients. Methods A total of 144 patients with T2DM newly diagnosed in our hospital from October 2019 to June 2021 were selected as the research objects, and they were divided into normal weight group (34 cases, BMI<24 kg/m2), overweight group according to body mass index (BMI). (38 cases, 24 kg/m2≤BMI<28 kg/m2),obese group (72 cases, BMI≥28 kg/m2). The expression level of plasma exosomes miR-155 and VFA, homeostasis model insulin resistance index (HOMA-IR) of each group were compared; The Pearson method was used to analyze the plasma exosomal miR-155 expression levels, VFA, HOMA-IR and clinical indicators, and the correlation between plasma exosomal miR-155 expression levels and VFA and HOMA-IR in obese T2DM patients; Multiple linear regression analysis of the influencing factors of plasma exosomal miR-155. Results The VFA and HOMA-IR in the obese group were higher than those in the normal weight group and overweight group (P<0.05), and the plasma exosomal miR-155 expression level was lower than that in the normal weight group and overweight group (P<0.05); the VFA and HOMA-IR in overweight group were higher than those in normal weight group (P<0.05), and the expression level of miR-155 in plasma exosomes was lower than that in normal weight group (P<0.05); in obese T2DM patients, BMI, FINS, waist-to-hip ratio (WHR) were negatively correlated with plasma exosomal miR-155 expression levels(r=-0.393, -0.406, -0.445, P<0.05), and positively correlated with VFA and HOMA-IR(r=0.456, 0.362, 0.401; 0.322, 0.387, 0.350; P<0.05); the expression level of plasma exosomes miR-155 in obese T2DM patients was negatively correlated with VFA and HOMA-IR (r=-0.588, -0.562, P<0.05);VFA and HOMA-IR were the influencing factors of miR-155 (P<0.05). Conclusion The expression level of plasma exosomes miR-155 in patients with obese T2DM is low, which is closely related to VFA and IR. MiR-155 may be a potential target for the prevention and treatment of obese T2DM.
