      目的 探讨婴儿痉挛症(IS)儿童脑网络特点及与促肾上腺皮质激素(ACTH)疗效的相关性。方法 选取2018年12月至2022年8月安徽省儿童医院确诊IS并给予ACTH治疗的20例患儿(IS组)为研究对象。另纳入同期20例健康儿童作为对照组。分析两组患儿脑电图数据,比较其脑网络属性并进行ACTH预后相关分析。结果 Sigma频段下,IS组特征路径长度(CPL)低于对照组,全局效率(GE)高于对照组,传递性(T)低于对照组,差异均有统计学意义(P<0.05)。IS组Pz电极中介中心性(BC)在alpha、sigma频段下小于对照组,差异有统计学意义(P<0.05)。进一步分析发现,Sigma频段下CPL越高、GE越低,ACTH短期疗效更好。结论IS患儿存在脑网络属性异常,CPL和GE可能成为评价IS患儿ACTH短期疗效的神经生物学标志物。

      Objective To discuss the characteristics of brain networks in children with infantile spasms and their correlation with the efficacy of adrenocorticotropic hormone treatment. Methods A retrospective analysis was conducted on the clinical data of 20 children, who were diagnosed with infantile spasms and treated with adrenocorticotropic hormone at Anhui Children’s Hospital between December 2018 and August 2022 (infantile spasms group). Additionally, 20 healthy children from the same period were included as a control group. Electroencephalography data from both groups were analyzed, their brain network properties were compared and adrenocorticotropic hormone prognosisrelated analysis was conducted. Results In the sigma frequency band, the characteristic path length in the infantile spasms group was lower than that in the control group, global efficiency was higher, and transitivity was lower, all showing statistically significant differences (P<0.05). The betweenness centrality of the Pz electrode in the infantile spasms group was lower than that of the control group in the alpha and sigma frequency bands, with statistically significant intergroup differences (P<0.05). Further analysis revealed that the higher characteristic path length and lower global efficiency in the sigma frequency band were associated with better short-term efficacy of adrenocorticotropic hormone. Conclusion Children with infantile spasms exhibit abnormal brain network properties.The characteristic path length and global efficiency may serve as neurobiological markers for evaluating the efficacy of adrenocorticotropic hormone in children with infantile spasms.
