      目的 探讨超声造影(CEUS)技术在关节滑膜增生穿刺活检中的应用价值,为临床诊疗提供参考。方法 选取2020年1月至2024年1月长沙市中心医院和中国科学技术大学附属第一医院(安徽省肿瘤医院)收治的65例CEUS技术引导穿刺活检的关节滑膜增生患者为CEUS引导组,另选取同期65例于长沙市中心医院和安徽省肿瘤医院行常规超声引导穿刺活检的关节滑膜增生患者为常规超声引导组。比较两组患者滑膜厚度、血流信号显示情况、穿刺取材次数、穿刺活检标本合格率、并发症情况,并分析CEUS引导组关节滑膜增生图像。结果 CEUS引导组增生滑膜厚度大于常规超声引导组,血流信号显示率高于常规超声引导组(P<0.05);两组患者检测滑膜血流分级情况比较,差异有统计学意义(P<0.05);CEUS引导组穿刺取材次数较常规超声引导组少(P<0.05);CEUS引导组穿刺标本合格率较常规超声引导组高(P<0.05)。两组患者均未出现并发症。CEUS引导组中有63例患者增生滑膜明显增强,其中高增强55例(87.30%),低或等增强8例(12.70%);有26例患者(41.27%)中心存在坏死区域。结论 CEUS技术引导下进行关节滑膜增生穿刺活检有利于明确观察滑膜增生情况,减少穿刺次数,提高穿刺标本合格率,CEUS图像可为临床诊疗提供重要影像学信息。

      Objective To explore the application value of contrast-enhanced ultrasound(CEUS) technology in the puncture biopsy of joint synovial hyperplasia, and provide reference for clinical diagnosis and treatment. Methods Sixty-five patients with joint synovial hyperplasia treated with CEUS guided puncture biopsy at Changsha Central Hospital and Anhui Provincial Cancer Hospital from January 2020 to January 2024 were selected as the CEUS guided group, while 65 patients with synovial hyperplasia treated with conventional ultrasoundguided puncture biopsy during the same period were selected as the conventional ultrasound-guided group. The synovial thickness, blood flow signal display, number of puncture sampling, qualified rate of puncture biopsy specimen and complications between the two groups of patients were compared, and the synovial hyperplasia images of the CEUS guided group were analyzed. Results The proliferative synovial membrane thickness of CEUS guided group was greater than that of conventional ultrasound guided group, and the blood flow signal display rate was higher than that of conventional ultrasound guided group(P<0.05). There is a statistically significant difference in synovial blood flow classification between the two groups of patients(P<0.05). The number of puncture sampling in CEUS guided group was less than that in conventional ultrasound guided group(P<0.05). The qualified rate of puncture specimens in CEUS guided group was higher than that in conventional ultrasound guided group(P<0.05). There were no complications in both groups. There were 63 cases of hyperplastic synovium enhancement in CEUS guided group, of which 55 cases(87.30%) had high enhancement, 8 cases(12.70%) had low or equal enhancement, and 26 cases(41.27%) had central necrotic area. Conclusion Puncture biopsy of joint synovial hyperplasia guided by CEUS technique is beneficial to clearly observe synovial hyperplasia, reduce the number of punctures, and improve the qualified rate of puncture specimens. CEUS images can provide important imaging information for clinical diagnosis and treatment.
