| 目的 探讨原发性甲状腺淋巴瘤的临床特征，提高临床诊断水平。 方法 回顾性分析在2013年至2017年间蚌埠医学院第一附属医院收治的，在超声引导下经粗针穿刺活检和病理证实的2例原发性甲状腺淋巴瘤患者临床和影像学资料，并进行相关文献复习。结果 2例患者均通过甲状腺彩超、粗针穿刺活检、免疫组化等检查途径诊断为原发性甲状腺淋巴瘤，2例患者均出现了颈部肿物的迅速增大并对气管有压迫症状（其中一例出现了声调变化）。结合文献复习，原发性甲状腺淋巴瘤是一种罕见病，超声引导的粗针穿刺活检对诊断原发性甲状腺淋巴瘤具有优势。结论 甲状腺肿短期内明显增大且质地坚硬患者应考虑甲状腺淋巴瘤可能; 超声引导下粗针穿刺活检和PET-CT检查是诊断原发性甲状腺淋巴瘤的有效手段, 可避免患者不必要的手术。 |

| Objective To investigate the clinical features of primary thyroid lymphoma and improve the clinical diagnostic level. Methods The clinical and imaging data of two patients with primary thyroid lymphoma confirmed by Core-needle biopsy and pathology admitted to the First Affiliated Hospital of Bengbu Medical College from 2013 to 2017 were retrospectively analyzed, and correlated literature was reviewed. Results Both patients were diagnosed as primary thyroid lymphoma by thyroid ultrasonography, core-needle biopsy and immunohistochemistry, both of them developed rapid enlargement of the neck mass and compression of the trachea (one of them had a tone change). Combining with literature review, primary thyroid lymphoma is a rare disease and Ultrasound guided rough needle biopsy has the advantage in the diagnosis of primary thyroid lymphoma. Conclusions The patients with significantly increased and hard goiter in short term should be considered to have possible thyroid lymphoma. Ultrasound-guided per cutaneous needle biopsy and PET-CT examination are effective methods to diagnose primary thyroid lymphoma and can avoid patients from unnecessary surgery. |
