      目的 探讨IPSe.max press铸瓷和氧化锆两种材料髓腔固位冠修复大面积缺损磨牙的临床效果。方法 选取83例患者经完善根管治疗后的100颗大面积缺损磨牙,随机分成两组,分别制作 IPSe.max press髓腔固位冠(铸瓷组)和氧化锆髓腔固位冠(氧化锆组)。在修复完成后的1周、6个月、1年、2年进行随访,按照改良美国公共卫生署(USPHS)标准对修复体进行评估。结果 铸瓷组在修复后6个月出现2例边缘轻度不密合,1例轻度牙龈反应,1例邻接关系略松;修复后1年出现3例边缘轻度不密合,2例轻度牙龈反应,1例修复体轻度折裂,2例邻接关系略松;修复后2年出现1例修复体折断和4例邻接关系略松。氧化锆组在修复后6个月出现2例边缘轻度不密合,1例轻度牙龈反应;修复后1年出现2例轻度牙龈反应,1例牙体折裂,1例邻接关系略松;修复后2年出现3例邻接关系略松。统计分析显示铸瓷组和氧化锆组数据之间差异无统计学差异(P>0.05)。结论 髓腔固位冠是修复磨牙大面积缺损的有效方法,IPSe.max press和氧化锆是临床常用的两种材质,用两者制作髓腔固位冠均能达到较好的修复效果。

      Objective To investigate the clinical effect of IPSe.max press and zirconia endocrown in repairing molars with large area defect. 100 molars with large defect after root canal treatment were selected from 83 patients and divided into two groups according to the height of the remaining axle wall. IPSe. Max press endocrown (cast porcelain group) and zirconia endocrown (zirconia group) were made respectively. Follow-up visits of 1 week, 6 months, 1 year and 2 years after the restoration were completed. The restoration was evaluated according to the modified American Public Health Service (USPHS) criteria. Results In group I, there were 2 cases of mild marginal incompatibility, 1 case of mild gingival reaction and 1 case of loosening of adjacent relationship 6 months after restoration, 3 cases of mild marginal incompatibility, 2 cases of mild gingival reaction, 1 case of mild fracture of prosthesis, 2 cases of loosening of adjacent relationship, 1 case of fracture of prosthesis and 4 cases of loosening of adjacent relationship 2 years after restoration. In group II, there were 2 cases of mild marginal incompatibility and 1 case of mild gingival reaction 6 months after restoration, 2 cases of mild gingival reaction 1 year after restoration, 1 case of tooth fracture, 1 case of loosening of adjacent relationship, and 3 cases of loosening of adjacent relationship 2 years after restoration. Statistical analysis showed that there was no significant difference between group I and group II (P > 0.05). IPSe. Max press and zirconia are two kinds of materials commonly used in clinic. Both of them can achieve good results in repairing molars with endocrown.
