      [摘要]目的 探究高分辨率磁共振成像（MRI）对直肠癌TN分期及肠壁外脉管侵犯的诊断价值。方法 选取阜阳市人民医院2020年1月至2020年12月收治的74例直肠癌患者的病历资料进行回顾性研究，均接受高分辨率MRI检测，并以病理检测作为直肠癌TN分期及肠壁外脉管侵犯诊断的金标准，采用Kappa一致性检验分析的方法比较高分辨率MRI与病理检测在直肠癌TN分期及肠壁外脉管侵犯中一致性。结果高分辨率MRI序列诊断直肠癌T2期、T3期、T4期的AUC分别为0.815、0.674、0.822（Kappa均＞0.40，P＜0.001）。高分辨率MRI序列诊断直肠癌N0期、N1期、N2期的AUC分别为0.753、0.773、0.804（Kappa均＞0.40，P＜0.001）。高分辨率MRI序列诊断肠壁外脉管侵犯的灵敏度、特异度、AUC、Kappa值分别为78.57%、88.33%、0.835（0.700～0.969）、0.634。结论高分辨MRI可用于直肠癌TN分期及肠壁外脉管侵犯的诊断，且诊断效能较高，与病理诊断一致性良好。

      【Abstract】Objective To explore the diagnostic value of high resolution magnetic resonance imaging (MRI) in TN staging of rectal cancer and vascular invasion outside the intestinal wall. Methods The medical records of 74 patients with rectal cancer admitted to Fuyang People's Hospital from January 2020 to December 2020 were selected for a retrospective study. All patients received high-resolution MRI testing. Pathological testing was used as the gold standard for the diagnosis of rectal cancer TN staging and extramural vascular invasion. Kappa consistency test analysis was used to compare the consistency of high-resolution MRI and pathological testing in the rectal cancer TN staging and extramural vascular invasion. Results The AUCs of T2, T3 and T4 were 0.815, 0.674 and 0.822 respectively (kappa > 0.40, P < 0.001). The AUCs of N0, N1 and N2 of rectal cancer diagnosed by high-resolution MRI sequence were 0.753, 0.773 and0.804 respectively (kappa > 0.40, P < 0.001). The sensitivity, specificity, AUC and kappa values of high-resolution MRI sequence in the diagnosis of extraintestinal vascular invasion were 78.57%, 88.33%, 0.835 (0.700 ~ 0.969) and 0.634, respectively.Conclusion High resolution MRI can be used in the diagnosis of TN staging and extraintestinal vascular invasion of rectal cancer, with high diagnostic efficiency and good consistency with pathological diagnosis.
