| 目的 分析81例声门上型喉癌并实施完整切除瘤体及双侧颈清扫术的患者预后疗效。方法 回顾性分析2016年1月～2018年10月期间手术治疗的声门上型喉鳞状细胞癌患者（n=81）的临床资料，采用SPSS26.0软件对本组喉癌患者的生存率及复发率进行分析评估。结果 81例患者在术中采取双侧颈淋巴结清扫术，cN+组（n=38）患者5年生存率为52.63%,局部复发率为7.89%；cN0组(n=43)pN+和pN0的5年生存率分别为52.94%和67.89%，pN+和pN0的局部复发率分别为6.67%和3.57%；术后并发乳糜漏1例。结论 声门上型喉癌患者尤其是CN0期患者，在保证完整切除瘤体的前提下进行双侧颈淋巴结清扫术可有效保证疗效，规范化双侧颈清扫可减少并发症的发生，提高患者生存质量。 |

| Objective；The efficacy and prognosis of patients with supraglottic laryngeal cancer after bilateral neck dissection were analyzed. Methods The clinical data of supraglottic laryngeal squamous cell carcinoma patients (n=81) from January 2016 to October 2018 were analyzed retrospectively, and the survival rate and recurrence rate of the laryngeal cancer patients were evaluated using SPSS26.0 software. Results In 81 patients with bilateral cervical lymph node dissection, the 5-year survival rate was 52.63% and 7.89%, 5-year survival rate in cN + (n=38), 5 2.94% and 67.89% for pN+and pN0, 6.67% and 3.57% for pN+ and pN0, respectively, and one postoperative chyle leak. Conclusion Prophylactic cervical lymph node dissection can effectively ensure the curative effect. The tissue structure should be clearly defined to reduce the occurrence of complications and improve the quality of life. |
