| 目的 探讨多层螺旋CT(MSCT)对腮腺基底细胞瘤的诊断价值,总结其具有诊断意义的CT征象。 方法 回顾性分析17例经手术和病理证实的腮腺基底细胞瘤的临床资料和CT资料,并复习相关国内外文献。 结果 腮腺基底细胞瘤中老年女性多见,多表现为腮腺区无痛性渐大性肿块。16例单发,1例多发;12个病灶位于浅叶,紧邻包膜下;15个病灶呈圆形、椭圆形,3个病灶呈浅分叶状;所有病灶均边界清楚;病灶较小,平均最大径约6~36 mm;9个病灶呈均匀或不均匀实性,增强后动脉期多明显强化,静脉期多缓慢持续强化或缓慢减退;9个病灶呈囊实性,增强后呈均匀薄壁"环形"、不均匀"花环状"或"附壁岛状"强化。 结论 腮腺基底细胞腺瘤具有一定的临床特点和CT表现,MSCT能够对大多数病灶做出准确诊断。 |

| Objective To evaluate the diagnostic value of MSCT on basal cell adenoma of the parotid gland, and summarize the significant CT signs. Methods The CT findings and clinical data of 17 patients with basal cell adenoma confirmed by operation and pathology were analyzed retrospectively, and the related literature were reviewed at home and abroad. Results Old women showed most basal cell adenoma of the parotid gland with manifestation of painless mass in parotid gland. 16 cases of basal cell adenoma were single, and 1 case was multiple; 12 lesions were located in the superficial lobe on the capsule of the gland; all the lesions had a clear margin; 15 lesions showed a round or round-like shape, and 3 lesions had a small lobular appearance; the diameters of the lesions ranged from 6 mm~36 mm; 9 lesions were solid nodules with an uniform or uneven density, at the arterial phase, the tumors showed a strong enhancement, and at the venous phase, the tumors showed a pattern of persistent strong enhancement or slow decline; another 9 lesions were solid lesions with a large cystic, and showed like a ring after enhancement, of which with an uneven thickness of cystic wall or papillary projection of the inner wall. Conclusion Basal cell adenoma of the parotid gland has a certain clinical characteristics and CT manifestation. MSCT can make an accurate diagnosis for the majority of lesions. |
