      目的 动态观察胃癌术后患者应用ω-3鱼油脂肪乳作为肠外营养(PN)支持的营养状态、免疫功能、炎症反应情况及肿瘤基因表达情况.方法 选择46例胃部肿瘤术后患者,随机分为试验组(20例)和对照组(26例).两组患者接受等氮、等热量PN支持,对照组添加单独的中/长链脂肪乳,试验组在给予中/长链脂肪的基础上添加ω-3鱼油脂肪乳.检测两组患者术前和术后7 d的营养指标、免疫功能和炎症指标.结果 手术前后,两组患者营养状态 (总蛋白、清蛋白、前清蛋白、视黄醇结合蛋白、转铁蛋白、总胆固醇)、肝肾功能(血清丙氨酸转氨酶、谷氨酸转移酶、肌酐、尿素氮)、肿瘤基因表达(血管内皮生长因子、胰岛素样生长因子)差异无统计学意义(P>0.05).试验组炎症指标水平(白细胞、C反应蛋白、白细胞介素-1β、白细胞介素-6、肿瘤坏死因子-α)较对照组明显降低,差异有统计学意义 (P<0.05). 结论 ω-3鱼油脂肪作为PN能减轻肿瘤患者术后的炎症反应,降低感染的发生率.

      Objective To investigate the effect of ω-3 fish oil fat emulsion-based parenteral nutrition (PN) on nutritional state, immune function, inflammatory reaction, expression of tumor factors and complication incidence in patients after surgical resection of gastric cancer. Methods Forty-six patients after surgical operation of gastric tumor in hospital were randomly divided into control group and intervention group. Patients in both groups were treated with isonitrogen and isocaloric parenteral nutrition support. In addition, the intervention group received ω-3 fish oil fat emulsion, while the control group received soybean oil. The indicators of nutrition, immune function and inflammation in the two groups were detected on the day before the operation and postoperative day 7. The rate of complications was compared between the two groups. Results There was no significant difference in nutritional state, liver function and renal function between the two groups (P>0.05). However, the levels of inflammatory markers were significantly decreased (P<0.05), and the rate of complication was also decreased i intervention group as compared with control group. Conclusion ω-3 fish oil fat emulsion-based parenteral nutrition alleviates the inflammatory reaction and reduces the rate of inflammatory complications.
