      目的 探讨防旋型股骨近端髓内钉(PFNA)在老年股骨粗隆间骨折伴骨质疏松症患者中的应用效果。方法 选择我院2008年2月至2015年2月收治的老年股骨粗隆间骨折伴骨质疏松症患者223例作为研究对象,根据不同手术方式分为2组,117例患者行PFNA组治疗(P组),106例患者行动力螺旋钉(DHS)治疗(D组),比较两组患者的围术期情况、治疗效果及术后并发症发生率。结果 P组切口长度、手术时间、术中出血量、骨折愈合时间、下地负重时间均低于D组,差异有统计学意义(P<0.05);P组术后并发症发生率为8.55%,显著低于D组的18.87%(P<0.05);P组优良率为95.73%,显著高于D组的87.74%,差异均有统计学意义(P<0.05)。结论 PENA治疗老年股骨粗隆间骨折伴骨质增生可有效减少术后并发症发生,有助于患者早期负重。

      Objective To investigate the application effect of proximal femoral nail antirotation(PFNA) in treatment of elderly patients with femoral intertrochanteric fracture and osteoporosis.Methods 223 elderly patients with osteoporosis and femoral intertrochanteric fracture, evertreated in our hospital from Feb 2008 to Feb 2015, were chosen as the research objects, and were divided into two groups according to surgicaloptions.117 cases underwent PFNA internal fixation(group P) and 106 cases fixed with dynamic hip screws(DHS)(group D), and the perioperative situation, therapeutic efficacy and postoperative complicationsbetween the two groups were compared.Results The incision length, operative time, blood loss, fracture healing time, weight-bearing time of patients in group P were lower than those in group D, with statistically significant differences(P<0.05).The incidence rate of complications after operation in group P was 8.55%, which was significantly lower than 18.87% in group D(P<0.05).The operation fineness rate of group P was 95.73%, which was significantly higher than 87.74% in group D(P<0.05).Conclusion The PFNA fixation in treatment of elderly patients with femoral intertrochanteric fracture and osteoporosis could efficiently reduce the postoperative complications, and be worthy of clinical application to help the patients to do weight-bearingexercises earlier.
