| 目的 分析安徽省结核病定点医院非耐药性肺结核患者诊断、治疗和管理现状。方法 采用多阶段分层抽样方法,在实施新型结核病防治服务体系的地区,根据结核病防控水平、地域分布、社会经济条件等选择6个地区,对2012年7月至2013年6月入院治疗的非耐药性肺结核患者进行问卷调查。结果 77.2%的调查对象是因本次患病才了解结核病相关知识;复治患者占16.7%,高于同期全省登记的复治患者比例(8.6%),差异有统计学意义(χ2=14.86,P<0.05);51.1%患者入院时有合并症或并发症;接受CT检查的比例(66.7%)高于接受传统X线胸片检查的比例(45.6%);40.6%患者住院费用超过了家庭年收入的10.0%。结论 患者在定点医院能接受更好的医疗服务,医疗负担较重,我省当前没有建立定点医院的补偿机制,新型结核病防治服务体系符合当前结核病防治形势需要。 |

| Objective To understand the current status of the diagnosis and treatment of non-drug-resistant patients in specialized hospitals in Anhui province.Methods According to tuberculosis prevention and control level, geographical distribution, economic conditions, six TB specialized hospitals in 6 cities were selected. A questionnaire survey was carried out in the patients with non-drug-resistant tuberculosis on July 2012 and June 2013.Results 77.2% of the investigation object understood the relevant knowledge because of this sickness. Retreatment proportion was 16.7% in hospitalized patients, which was higher than in registered active patients in the same period(8.6%) in Anhui province, and the difference was statistically significant(χ2=14.86, P<0.05). 51.1% of patients admitted to hospital had complications. The proportion of accepting CT examination(66.7%) was higher than that of the conventional X-ray examination(45.6%). The hospital costs of 40.6% of patients were more than 10% of their annual household income.Conclusion Patients can receive better medical services in specialized hospitals, but the medical burden is heavier. Current compensation mechanism is to be established in specialized hospitals in the province. The new tuberculosis control service system is in line with the requirement of current TB control situation. |
