      目的 观察不同剂量氯沙坦对糖尿病肾病(DN)患者尿中转化生长因子-1(TGF-1)及8-羟基脱氧鸟嘌呤(8-OHdG)的影响。方法 收集48例有明显蛋白尿的2型DN患者,给予氯沙坦50 mg/d治疗8周,之后改为100 mg/d治疗8周,比较治疗前后患者尿TGF-1、8-OHdG水平变化。采用Spearman分析患者尿TGF-1、8-OHdG浓度变化与24 h尿蛋白变化的相关性。结果 患者治疗前、50 mg/d治疗后、100 mg/d治疗后,新鲜尿TGF-1、24 h尿8-OHdG的浓度明显下降,差异有统计学意义(P<0.05)。尿TGF-1浓度变化与尿蛋白变化具有相关性(P<0.01)。所有患者对氯沙坦100 mg/d耐受性良好。结论 氯沙坦能显著降低2型DN患者尿TGF1、8-OHdG水平,100 mg/d比50 mg/d作用更强,而且副作用无明显增加。

      Objective To observe the effects of different doses of Losartan on urinary transforming growth factor-β1(TGF-1) and 8-hydroxy-2'-deoxyguanosine(8-OHdG) excretion in the patients with diabetic nephropathy(DN). Methods 48 cases of type 2 DN patients with macro-albuminuria were enrolled in the study, and all patients then received Losartan 50 mg/d for 8 weeks, followed by Losartan 100 mg/d for additional 8 weeks. Changes in the urinary concentrations of TGF-β1 and 8-OHdG before and after the drug treatment were compared. The relation between the concentration changes of urinary TGF-β1 and 8-OHdG and the amount changes of 24-hour urine protein was analyzed with Spearman correlation analysis. Results There was an apparent reduction of urinary TGF-β1 and 8-OHdG excretion at the time of before treatment and after the Losartan therapy of 50 mg/d and 100 mg/d, with statistically significant differences(P<0.05). Obvious correlation between the changes of urinary TGF-β1 levels and urine protein was found(P<0.01). During the treatment course, 100 mg/d Losartan was well tolerated by all patients. Conclusion Losartan therapy of 50 mg/d and 100 mg/d could significantly reduce the urinary TGF-β1 and 8-OHdG excretion in the type 2 DN patients, and the effect of Losartan 100 mg/d is more obvious without increase of side effect.
