| 目的 探讨胎盘植入的MRI征象及其诊断价值。方法 回顾性分析16例经手术证实的胎盘植入患者的临床资料,其中6例为产前行MRI检查,10例为产后。所有患者均行MRI平扫,其中2例同期行增强扫描,总结其MRI征象。结果 16例手术证实为胎盘植入的患者,MRI诊断15例(15/16,93.75%),6例产前MR示胎盘形态欠规则,与子宫交界面不清;10例产后MR表现为胎盘植入呈不均匀长T1、长或混杂T2信号,内见点、条状短T2信号;与邻近正常子宫壁分界不清;子宫浆膜层信号中断;子宫壁或宫旁血管增多;病灶内伴短T1出血信号;2例增强后胎盘明显强化。结论 MRI对胎盘植入有重要诊断价值。 |

| Objective To study the MRI imaging findings of placental implantation abnormality(PIA) and its diagnostic value. Methods The clinical data of 16 cases of PIA patients identified by surgery were retrospectively analyzed, in which 6 PIA cases underwent antenatal MRI scans and the other 10 cases with postpartum MRI scans. All patients underwent MRI plain scans and 2 of them with further enhanced scans at the same time, then the MRI imaging findings of these PIA patients were summarized and analyzed. Results In 16 patients with placenta implantation identified by surgery, 15 cases were consistent with MRI diagnosis(15/16, 93.75%). 6 cases of antenatal MRI appearance showed irregular shape of placenta and unclear boundary with muscular layer of uterine. 10 cases of postpartum MRI appearance of PIA showed inhomogeneous long T1WI, long or mixed T2WI signal, with inner short dot-bar T2WI signal, unclear boundary with adjacent normal uterine wall, incomplete signal of serosa layer of uterine, increased number of vessels in the uterine wall or beside the uterine, short T1WI signal of bleeding in the lesions. 2 cases of enhanced MRI scans showed obvious enhanced appearance of placenta. Conclusion The MRI placenta scans have important diagnostic value for PIA. |
