| 目的 探讨应用闭合复位及股骨近端防旋髓内钉(PFNA)内固定治疗老年股骨转子间骨折的临床疗效。方法 采用PFNA治疗老年股骨转子间骨折患者64例,并按Harris评分标准评价术后髋关节功能。结果 所有骨折均愈合,骨折愈合时间10~21周,平均14.2周。术后肺部感染3例,髋内翻畸形2例。髋关节功能恢复程度参照Harris评分:优38例,良21例,可4例,差1例,优良率92.1%。结论 PFNA具有手术时间短、创伤小、术中出血少、固定可靠和并发症少等优点,是一种有效治疗老年股骨转子间骨折的方法。 |

| Objective To discuss the clinical efficacy of closed reduction and inner fixation with proximal femoral nail anti-rotation(PFNA) in the treatment of femoral intertrochanteric fractures in the elderly patients. Methods 64 cases of elderly patients with femoral intertrochanteric fractures were treated with PFNA, and the postoperative hip joint function was evaluated by the Harris scores. Results The mean fracture healing time was 14.2 weeks(range from 10 to 21 weeks), with a healing rate of 100%. There were 3 cases of pulmonary infection and 2 cases of coxa vara deformity after inner fixation. According to the Harris hip scores, the results of hip joint recovery were defined as excellent in 38 cases, good in 21 cases, fair in 4 cases, fail in 1 case, and the excellent and good rate was 92.1%. Conclusion Inner fixation with PFNA has the advantage of short operation time, less operative trauma and bleeding, reliable fixation and few complications, and could be applied as an effective therapy for femoral intertrochanteric fractures in the elderly. |
