| 目的 评估人工晶体(IOL)植入术后YAG激光联合氩激光治疗伴虹膜后粘连人工晶体前膜的疗效。方法 对人工晶体前膜伴虹膜粘连患者55例(59眼),使用Nd:YAG激光联合氩激光方法切除人工晶体前表面机化膜,对合并虹膜粘连者予以松解,观察治疗后患者视力提高情况、一次性激光治疗成功率、人工晶体的损伤、术后人工晶体前膜再发率及术后并发症。结果 57眼一次性激光治疗后前膜全部切除且解除虹膜与人工晶体间粘连,2眼因前房炎症反应较重复发,行第二次激光治疗后痊愈,激光治疗一次成功率达96.6%。无严重并发症发生。随访6个月所有患者瞳孔区人工晶体前膜完全切除,虹膜粘连松解。视力稳定提高≥3级者51眼(86.4%)。结论 YAG激光联合氩激光切除人工晶体前膜、松解虹膜后粘连的方法安全有效,并发症少,可及早恢复视力。 |

| Objective To evaluate the efficacy of Nd:YAG laser combined with Argon laser in the treatment of membrane forming on the anterior surface of intraocular lenses(IOL) with iris posterior synechia after IOL implantation. Methods 55 outpatients(59 eyes) with anterior fibrous membrane of IOL with iris posterior synechia were treated with Nd:YAG laser combined with Argon laser. After the fibrous membrane on anterior surface of IOL was cleared and the posterior synechia of iris was released, these indexes of visual acuity recovery, success of one-time laser therapy, damage of IOL, recurrence of anterior membrane of IOL, and complications after treatment were observed in all patients. Results The anterior fibrous membrane of IOL and posterior synechia in 57 eyes were completely removed in the first laser treatment. Only 2 eyes needed the second laser treatment because of recurrence of the fibrous membrane of IOL due to severe anterior chamber inflammation. There was no serious complication after the first laser treatment with one-time success rate of 96.6%. After following up for 6 months, the anterior fibrous membrane of IOL and posterior synechia remained completely clear in the pupillary zone of all patients. Their visions gradually improved with no less than 3 grades in 51 eyes(86.4%). Conclusion Application of Nd:YAG laser combined with Argon laser in clearing the membrane on anterior surface of IOL and releasing the iris posterior synechia is safe and effective for early visual recovery, with less complication. |
