      目的 检测4种微小RNA(miRNA)含量对于诊断肝细胞性肝癌(HCC)的临床价值。方法 连续性纳入2013年12月至2015年7月所收治的HCC患者为试验组,肝硬化及肝血管瘤患者为对照组。比较两组患者各项血清肿瘤标志物及miRNA-18、miRNA-21、miRNA-122、miRNA-192含量之间的差异性,并使用logistic回归分析进一步提取独立敏感指标,绘制相应指标的ROC曲线。结果 试验组共纳入46例符合要求的HCC患者,对照组纳入肝血管瘤患者53例、肝硬化患者40例。3组间资料比较显示:癌胚抗原(CEA)、肿瘤特异性生长因子(TSGF)、α-L-岩藻糖苷酶(AFU)、甲胎蛋白(AFP)、癌抗原242(CA242)、miRNA-21、miRNA-192共7项指标存在差异(P均<0.05);多因素logistic回归分析显示AFU、AFP、TSGF、miRNA-21、miRNA-192为区分HCC患者与其他患者的独立危险因素(P均<0.05);其中miRNA-21与AFU存在正相关(r=0.621,P<0.05)、与AFP存在正相关(r=0.819,P<0.05);miRNA-192与AFU存在正相关(r=0.650,P<0.05)、与TSGF存在正相关(r=0.598,P<0.05);AFU、AFP、TSGF、miRNA-21、miRNA-192对于诊断HCC患者均具有较好的区分价值(AUC均>0.7);AFU、AFP、TSGF联合诊断HCC的ROC AUC为0.850;将miRNA-21、miRNA-192纳入上述联合诊断体系,AUC增加至0.881。结论 血清miRNA-21、miRNA-192对于HCC具有良好的诊断价值,与部分临床常用的HCC肿瘤标志物之间存在良好的线性相关趋势,将其纳入当前诊断体系有助于提升HCC的诊断效能。

      Objective To understand the clinical value of detecting four kinds of microRNA(miRNA) for the diagnosis of hepatocellular carcinoma(HCC). Methods HCC patients who received the hospital treatment from December, 2013 to July, 2015 were recruited into the test group continually. The cirrhosis and liver hemangioma patients in the same period were recruited into the control group. The differences between the two groups in serum tumor markers and miRNA-18, miRNA-21, miRNA-122, miRNA-192 were contrasted. Multivariate Logistic regression analysis was used to extract sensitive indicators, and the Receiver operating characteristic curve(ROC curve) about these indicators was drawn. Results 46 cases of HCC patients were recruited into the experimental group, 53 cases of hepatic hemangioma patients and 40 cases of liver cirrhosis patients were recruited into the control group. Comparison of data showed that seven indicators were significantly different(P<0.05):carcinoembryonic antigen(CEA), tumor specific growth factor, (TSGF), alpha-L-fucosidase(AFU), alpha-fetoprotein(AFP), cancer antigen 242(CA242), miRNA-21, miRNA-192. Multivariate Logistic regression analysis showed that these risk factors could be used to distinguish HCC patients from other patients(P<0.05):AFU, AFP, TSGF, miRNA-21, miRNA-192. Significant positive correlation was present between miRNA-21 to AFU(r=0.621, P<0.05) and miRNA-21 to AFP(r=0.819, P<0.05);Also, markedly positive correlation was present between miRNA-192 to AFU(r=0.650, P<0.05) and miRNA-192 to TSGF(r=0.598, P<0.05). These indicators had value in the diagnosis of HCC patients(AUC were>0.7):AFU, AFP, TSGF, miRNA-21, miRNA-192. ROC AUC of HCC by combined diagnosis of AFU, AFP, TSGF was 0.850;then, miRNA-21, miRNA-192 were included in the above diagnostic system, and the AUC was increased to 0.881. Conclusion miRNA-21 and miRNA-192 have good diagnostic value of HCC;together with common tumor markers, these mRNAs have good correlation and can promote the current system of the diagnosis of HCC.
