| 目的 探讨多通道经皮肾镜气压弹道超声联合碎石术（PCNL）在复杂性肾结石治疗中的临床应用价值。方法 回顾分析安徽医科大学第二附属医院2013年7月至2016年5月收治的272例采用多通道PCNL治疗的复杂性肾结石患者临床资料，分析总结PCNL的结石处理时间、结石清除率及并发症发生情况。结果 所有病例均在超声引导下成功建立2个24F经皮肾通道，结石处理时间20～86 min，Ⅰ期结石清除率83%，Ⅱ期结石清除率92%，无Clavien Ⅲ级及以上的并发症发生。结论 多通道PCNL治疗复杂性肾结石可缩短手术时间，提高结石清除率，降低术后并发症，在成熟的泌尿外科中心具有较高的临床价值。 |

| Objective To investigate the clinical applicationvalueof multi-channel percutaneous nephrolithotripsy(PCNL) combinedwithpneumatic and ultrasound power in the treatment ofcomplex renal calculi.Methods The clinical data of 272 patients with complex renal calculiunderwentmulti-channel PCNL from Jul 2013 to May 2016 in ourhospitalwereretrospectively analyzed. The operation time, clearance rate and complications of multi-channel PCNL in all cases weresummarized and studied. Results In all cases,two 24F percutaneous renal accesseswere successfully establishedbyultrasoundguidance. Theoperation timeof calculimanagementwaswithin20~86minutes,and the clearance rateof renalcalculiwas 83%in phaseI and92%in phaseII, withno complicationofClavien III~ IV.Conclusion Application of multi-channel PCNLcombined with pneumatic and ultrasound powercouldshorten operation time, improve efficacy of calculi clearanceand decrease postoperative complicationsin the treatment of complex renal calculi, and would be more suitable and profitablefor a superior Urology Centerwith high technology maturation. |
