| 目的 探讨超声检查在植入性凶险型前置胎盘（PPP）诊断中的价值。方法 选择2010年1月至2014年12月期间因植入性PPP在新津县中医院妇产科或成都市西区医院妇产科住院治疗的23例孕妇为研究对象。患者均给予产前超声检查，回顾分析超声图像特征，并随访临床结果。结果 在23例植入性PPP中，超声检查正确诊断20例，准确率为87.0%（20/23），漏诊3例，漏诊率约为13.0%（3/23）。声像图特征：PPP伴有胎盘附着处子宫肌层变薄消失、胎盘和肌层分界不清、胎盘实质内腔隙血流形成、胎盘植入处肌壁涡旋样血流、膀胱和子宫肌层间分界不清等征象。结论 超声检查诊断植入性PPP具有准确率较高，可重复性好的优点，对产前明确诊断具有重要的临床价值。 |

| Objective To explore the application value of ultrasonography in the diagnosis of implanted pernicious placenta previa (PPP).Methods From Jan 2010 to Dec 2014, 23 cases of pregnant patients with implanted PPP ever hospitalized in our hospitals were enrolled as the research objects.All patients were detected by the prenatal ultrasonography, and their ultrasonic image characteristics were reviewed and followed up.Results Among the 23 patients with implanted PPP, 20 cases were correctly diagnosed with an accuracy rate of 87%, and other 3 cases were misdiagnosed.The features of ultrasonic images included such signs as PPP associated thin and even disappeared myometrium at the placental attachment, unclear boundary between the placenta and myometrium, flow formation within gaps of placental parenchyma, vortex flow at the muscular wall of placenta increta, unclear boundary between the bladder and myometrium.Conclusion Application of ultrasonography in the diagnosis of implanted PPP has the advantages of higher accuracy and better repeatability, and has important clinical value for the prenatal diagnosis of PPP. |
