      目的 探讨妊娠期肝内胆汁淤积症(ICP)患者胎盘组织瘦素mRNA的表达及其临床意义。方法 采用实时荧光定量(RTQ-PCR)法检测2014年3月至2016年3月安庆市立医院收治的30例ICP孕妇和12例正常妊娠孕妇胎盘瘦素mRNA表达水平,其中ICP患者按不同临床分度、不同孕周分类。结果 ①瘦素mRNA在42例胎盘组织中均有表达;②15例轻度ICP组瘦素mRNA水平(0.220 6±0.170 1)U/μL、15例重度ICP组(0.176 9±0.147 0)U/μL及12例正常妊娠组的瘦素mRNA水平(0.154 5±0.177 3)U/μL相比,差异无统计学意义(P>0.05);③13例ICP足月产组瘦素mRNA水平(0.285 5±0.173 4)U/μL、17例ICP早产组为(0.132 5±0.108 7)U/μL、正常妊娠组为(0.154 5±0.177 3)U/μL,3组比较差异有统计学意义(F=4.121,P=0.024),但两两比较,ICP早产组与正常妊娠组差异无统计意义(P>0.05)。结论 瘦素在足月产ICP患者中呈高表达趋势,缺氧可能是主要原因,胎盘组织中瘦素mRNA表达水平在一定程度上反应了胎盘功能,可能参与ICP发病。

      Objective To investigate the leptin-mRNA in placental tissues of patients with intrahepatic cholestasis of pregnancy(ICP). Methods Forty-two patients were selected in the hospital at random of 30 patients with ICP and 12 normal pregnant women.Patients with ICP were divided into two groups according to different gradinganddifferent gestational age.Real-time fluorescent quantitative (RTQ-PCR)was used to detect human leptin-mRNA derived from placenta.Statistical data were analyzed by SPSS 20.0. Results ①Leptin -mRNA in 42 cases of placenta tissues was all expressed.②The difference inleptin mRNA levelwas not statistically significant in 15 cases of mild ICP group[(0.220 6±0.170 1)U/μL], 15 cases of severe ICP group[(0.176 9±0.147 0)U/μL] and 12 cases of normal pregnancy group[(0.154 5±0.177 3)U/μL](P>0.05).③The expression level of placental leptin-mRNA of 13 full-term patients with ICP[(0.285 5±0.173 4)U/μL]was higher than that in 17 premature delivery group[(0.132 5±0.108 7)U/μL] and the normal group[(0.154 5±0.177 3)U/μL],withstatistically significant difference (F=4.121,P=0.024). The others showed no significant difference in ICP premature delivery group and the normal group (P>0.05). Conclusion The high expression of placental leptin-mRNA in patients with ICP reflects the function of placenta and the main reason may be hypoxia.It may participate in the onset of ICP.
