      目的 探讨134例不同病变肝组织中热休克蛋白27(HSP27)的表达情况及与HBsAg的相关性,探寻能否运用HSP27提高肝细胞癌(HCC)的早期诊断率。方法 收集昆明医科大学病理教研室和昆明医科大学第二附属医院病理科2004~2014年存档蜡块共134份,应用免疫组化SP法检测HSP27和HBsAg在不同肝组织中的表达情况。结果 ①120份不同病变肝组织中HSP27在HBsAg阳性表达中的数值为3.89±3.16,在HBsAg阴性表达中的数值为2.38±2.56,两者差异无统计学意义(t=-1.200,P=0.241);②HSP27的表达水平从高到低为HCC > 癌周肝硬化 > 肝硬化 > 慢性肝炎和正常肝组织,差异有统计学意义(P<0.05)。结论 不同病变肝组织中HSP27的表达水平与乙肝病毒感染无相关;HSP27在HCC中高表达,可作为HCC的早期诊断标志物。

      Objective To explore the expression of heat shock protein 27 in 134 cases of different pathological changes of liver tissue and the relevance to hepatitis B surface antigen, exploring whether heat shock proteins 27can improve the early diagnosis rate of HCC. Methods A total of 134 paraffin tissue blocks were collected from the year 2004to 2014, which came from pathology teaching and research section of both Kunming Medical University and the Second Affiliated Hospital of Kunming Medical University. Immunohistochemical SP methodwas used to detect heat shock protein 27 and hepatitis B surface antigen in different expression and correlation of liver tissues. Results ①In 120 cases of differentpathological changes of liver tissues, there was no statistical significance between the expression of hepatitis B surface antigen and the expression level of heat shock protein 27(t=-1.200,P=0.241); ②The expression level of heat shock protein 27 of hepatocellular carcinoma > paracancerous cirrhosis > liver cirrhosis > chronic hepatitis and normal liver. There was significant statistical significancein the overall differences. Conclusion It can not be concluded that there is association between the expression ofheat shock protein 27 of different liver tissues with hepatitis B virus infection. Heat shock protein 27 high expression can be used as a diagnostic marker for early diagnosis of hepatocellular carcinoma.
