      目的 探讨应用筏钉技术结合3D打印治疗伴有关节面塌陷的胫骨平台骨折的临床疗效。方法 选择2013年9月至2015年9月铜陵市立医院骨科收治的41例伴关节面塌陷的胫骨平台骨折患者,均采用筏钉技术支撑塌陷关节面,41例患者按不同的术前准备方式分为3D组(16例)和普通组(25例)。比较两组手术时间、术中出血量、术中骨折一次性复位成功率、术后膝关节功能评分和关节面二次塌陷率等。结果 3D组患者手术时间、术中出血量均小于普通组,差异有统计学意义(P<0.05);3D组患者术中骨折一次性复位成功率和术后Rasmussen膝关节功能评分高于普通组,差异有统计学意义(P<0.05),术后9个月两组患者关节面二次塌陷率的差异无统计学意义(P>0.05)。结论 筏钉技术结合3D打印治疗伴有关节面塌陷的胫骨平台骨折,可减少手术时间和术中出血量,提高术中骨折复位效率,改善术后膝关节功能。
英文摘要:
      Objective To investigate the clinical effectiveness in treating articular-depression tibial plateau fractures by using the rafting screws and 3D printing. Methods The clinical data of 41 patients with articular-depression tibial plateau fractures treated by rafting screws in this hospital between September 2013 and September 2015 were retrospectively analyzed. According to the different preoperative preparations, they were divided into 3D group(16 cases) and traditional group(25 cases). Comparison between the two groups were made in operation time, blood loss, successful fracture reduction rate, knee function score and articular second depression. Results The operation time and blood loss of group 3D were significant smaller thanthose of traditional group (P<0.05); group 3D also performed better than traditional group in comparison of successful fracture reduction rate and the Rasmussen knee function score(P<0.05); there was no statistical differences in articular second depression between the two groups after 9 months' follow-up (P>0.05). Conclusion In the treatment of articular-depression tibial plateau fractures, the technique of rafting screws and 3D printing can reduce the operation time and blood loss, improve the efficiency of fracture reduction and the knee function.
