| 目的 对比应用腹腔镜技术与传统开腹治疗进展期胃癌的安全性及短期疗效。方法 选择铜陵市人民医院2009年1月至2015年12月进行手术治疗的273例进展期胃癌患者，根据手术方式分为两组，92例行腹腔镜下胃癌D2根治术为腔镜组，181例行开腹胃癌D2根治术为开腹组，对比分析两组患者术中及术后康复指标（术中出血量、手术时间、术后胃肠功能恢复时间、术后住院时间）、手术并发症率及手术效果指标（淋巴结清扫数目、肿瘤远近切缘距离）。结果 腹腔镜组与开腹组相比，全胃切除手术时间、术后胃肠功能恢复时间、术后住院时间、术后并发症发生率的差异有统计学意义（P<0.05），远端胃切除时间、术中出血量、淋巴结清扫数目及肿瘤远近切缘距离的差异无统计学意义（P>0.05）。结论 腹腔镜技术与开腹手术治疗进展期胃癌的短期疗效与手术风险无明显差异，腹腔镜手术具有术后恢复快、并发症少的优势。 |

| Objective To compare the safety and therapeutic efficacy of laparoscopic radical resection and laparotomy for advanced gastric cancer.Methods A retrospective study was performed on 273 patients with advanced gastric cancer, among whom92 patients received laparoscopic assisted D2 radical gastrectomy(laparoscopic group) and 181 patients received laparotomy(laparotomy group).Intraoperative and postoperative indicators (intraoperative blood loss, operation time, recovery time of postoperative gastrointestinal function and postoperative hospital days), postoperative complications and operative efficacy indicators (number of lymph node excision and distance from the cut edge of the tumor) between the two groups were compared.Results There were significant differences (P<0.05) in time for total gastric resection, recovery time of postoperative gastrointestinal function, postoperative hospital days and postoperative complications between laparoscopic group and laparotomy group. No significant differences (P>0.05) were observed in time for distal gastric resection, intraoperative blood loss, number of lymph node excision and distance from the cut edge of the tumor between the two groups. Conclusion Laparoscopic radical gastrectomy could be considered as a safe and effective treatment for advanced gastric cancer with less complications and faster recovery compared with laparotomy. |
