      目的 检测尿沉渣中尿路上皮细胞miR-193a、miR-3189、miR-22、miR-34a、miR-124-1甲基化状态,评估用于诊断膀胱癌的临床意义。方法 收集2014年6月至2016年6月安徽省立医院泌尿外科96例术前膀胱癌和62例正常对照组患者中段晨尿50~100 mL并提取尿沉渣尿路上皮细胞DNA,利用甲基化特异性PCR(MSP)检测尿沉渣尿路上皮细胞DNA中miR-193a、miR-3189、miR-22、miR-34a、miR-124-1甲基化状态,对比膀胱癌患者和正常对照组尿成渣miRNA甲基化状态差异。结果 miR-193a、miR-3189、miR-22、miR-34a、miR-124-1在膀胱癌患者尿沉渣中甲基化阳性率分别为88.54%、80.21%、60.42%、36.46%、42.71%,正常对照组分别为9.68%、14.52%、29.03%、22.58%、27.42%;miR-193a诊断膀胱癌敏感性和特异性分别为88.54%、90.32%,联合miR-3189、miR-22、miR-34a、miR-124-1敏感性和特异性分别达到97.92%、74.19%。结论 联合检测miR-193a、miR-3189、miR-22、miR-34a、miR-124-1在膀胱癌患者尿沉渣中的甲基化状态能提高膀胱癌诊断的敏感性,可能成为膀胱癌无创诊断的潜在方法。

      Objective To detect the methylation status of miR-193a, miR-3189, miR-22, miR-34a and miR-124-1 in tract epithelial cells ofurinary sediment and evaluate the clinical significance of bladder cancer diagnosis. Methods All patients were divided into two groups:one consisted of 96 bladder cancer patients and the other recruited 62 non-neoplastic patients with urinary system disease.We collected tract epithelial cells of urinary sediment DNA from approximately 50 to 100 mL preoperative urine. Then the methylation specific PCR was used to detect the differentiation methylation status of miR-193a, miR-3189, miR-22, miR-34a and miR-124-1 in two group sediment. Results The methylation positive frequency of miR-193a, miR-3189, miR-22, miR-34a, miR-124-1 in bladder cancer sediment was 88.54%, 80.21%, 60.42%, 36.46%, 42.71%, respectively, while in normal control group it was 9.68%, 14.52%, 29.03%, 22.58%, 27.42%, respectively. The diagnostic sensitivity and specificity of miR-193a was 88.54% and 90.32%,respectively, the sensitivity and specificity added to 97.92% and 74.19% when combine all five miRNAs.Conclusion Methylation status of miR-193a, miR-3189, miR-22, miR-34a and miR-124-1 in urinary sediment can improve the sensitivity and may be a noninvasive potential target for the diagnosis of bladder cancer.
