      目的 研究窒息新生儿肾功能损害相关指标的变化情况。方法 选择河南省郑州市第一人民医院2013年4月至2015年4月收治的80例窒息新生儿为研究对象,并作为窒息组。其中,轻度窒息50例,重度窒息30例;选择同期40例正常新生儿为对照组。通过全自动生化分析仪、颗粒增强透射免疫比浊法分别测定各组新生儿血肌酐(Cr)、尿素氮(BUN)及血清胱抑素C (CysC)水平,并用Pearson分析CysC与BUN及Cr的相关性。结果 窒息组BUN、Cr、CysC水平分别为(7.52±2.04)mmol/L、(113.61±29.65)μmol/L、(4.23±0.71)mg/L,高于对照组的(3.91±1.20)mmol/L、(59.63±15.60)μmol/L、(2.54±0.40)mg/L,差异均具有统计学意义(P<0.05)。轻度窒息组CysC、BUN异常率分别为74.0%、42.0%,低于重度窒息组的96.7%、66.7%,差异有统计学意义(P<0.05)。CysC与血Cr、BUN呈正相关(r=0.670、0.654,P<0.05)。结论 血清BUN、Cr及CysC,特别是CysC水平,对于判断窒息新生儿肾功能损害有重要意义。

      Objective To study the changes of renal function damage associated indicatorsin asphyxiated newborns. Methods Eighty cases of asphyxiated newborns treated in the First People's Hospital of Zhengzhou City in Henan from April 2013 to April 2015 were selected as the study subjects (asphyxiated group), including 50 cases with mild asphyxia and 30 cases with severe asphyxia. Forty cases of normal newborns were selected as the control group at the same time. Levels of serum creatinine(Cr), blood urea nitrogen (BUN) and serum Cystatin C (CysC) were determined usingthe automatic biochemical analyzer and particle-enhanced turbidimetric immunoassay. The correlation of CysC with BUN and Cr was analyzed by Pearson correlation analysis.Results The levels of BUN, Cr and CysC inasphyxiated group[(7.52±2.04) mmol/L, (113.61±29.65) μmol/L and (4.23±0.71) mg/L] were significantly higher than those in control group[(3.91±1.20) mmol/L, (59.63±15.60) μmol/L and (2.54±0.40) mg/L] (P< 0.05)].The abnormal rates of CysC and BUN inmild asphyxia group (74.0% and 42.0%) were significantly lower than those insevere asphyxia group (96.7% and 66.7%) (P< 0.05).CysC was correlated with Cr and BUN (r=0.670, 0.654, P< 0.05). Conclusion The detection of serum BUN, Cr and CysC, especially the CysC level, is of very great significance in judgement of renal function damage in asphyxiated newborns.
