      目的 探讨肺鳞癌组织中胰岛素样生长因子1(IGF-1)和趋化因子20(CCL20)的表达与患者临床病理特征及预后的相关性。方法 选取2014年2月至2015年2月皖北煤电集团总医院收治的肺鳞癌患者30例,采用免疫组织化学染色法检测患者肺鳞癌组织中IGF-1和CCL20蛋白的表达,通过单因素分析IGF-1和CCL20蛋白与患者临床病理特征的相关性;采用单因素和多因素Cox比例风险模型分析其对患者预后的关系。结果 30例肺鳞癌患者中,18例(60.00%)患者IGF-1蛋白呈阳性表达,且与患者TNM分期及淋巴结转移有关(P<0.05);23例(76.67%)患者CCL20呈阳性表达,且与患者淋巴结转移、浸润深度、TNM分期有关(P<0.05)。经单因素和多因素Cox比例风险模型分析,浸润深度、淋巴结转移、TNM分期、IGF-1、CCL20阳性表达是影响肺鳞癌患者3年生存期的独立危险因素(P<0.05)。结论 IGF-1及CCL20高表达与肺鳞癌的侵袭转移密切相关,可作为判定肺鳞癌患者预后的指标之一。

      Objective To analyze the expression of insulin-like growth factor-1 (IGF-1) and chemokine (C-C motif) ligand 20 (CCL20) proteins in the lung squamous carcinoma tissues and their relationships with the clinicopathologic features and prognosis of cancer patients. Methods 30 patients with lung squamous carcinoma ever treated in our hospital between Feb 2014 and Feb 2015 were selected, and expression of IGF-1 and CCL20 proteins in their lung carcinoma samples were detected by means of immunohistochemistry staining. Univariate variance analysis was then performed to identify the relationship between expression of IGF-1 and CCL20 proteins and clinicopathologic features of these cancer patients. Furthermore, univariate and multivariate analysis with Cox proportional hazard model were used to evaluate the influencing factors of their prognosis. Results Among the 30 patients with lung squamous carcinoma, 18 patients (60.0%) showed positive expression of IGF-1 protein and its correlation with the TNM stage and lymph node metastasis (P<0.05); 23 patients (76.67%) showed positive expression of CCL20 protein and its correlation with the lymphatic metastasis, infiltration depth and TNM stage (P<0.05). Univariate and multivariate analysis with Cox proportional hazard model indicated that, infiltration depth, lymphatic metastasis, TNM stage, and positive expression of IGF-1 and CCL20, were all the independent risk factors influencing three-year survival time of patients with lung squamous carcinoma (P<0.05). Conclusion Higher expression of IGF-1 and CCL20 might be closely related with the invasion and metastasis of lung squamous carcinoma, and could be used as a potential diagnostic factor of prognosis in those patients with lung squamous carcinoma.
