      目的 观察支气管套囊充气法在右侧双腔管(DLTs)插管定位中的成功率。方法 选择2016年6月至2017年12月在安徽医科大学第一附属医院胸外科住院的46例患者,均为适合右侧DLTs插管的胸外科手术患者。应用随机数字表法将患者分为I组与Ⅱ组,每23例。I组的右侧DLTs定位采用支气管套囊充气法,Ⅱ组患者以听诊法定位。收集DLTs插管定位和纤维支气管镜(FOB)检查过程的视频,收集并比较两组患者DLTs的位置情况、气管插管时间以及FOB检查和纠正DLTs位置的时间。结果 I、Ⅱ组患者的插管时间分别为(103.22±18.34)s和(99.39±16.31)s,两组差异无统计学意义(P>0.05);I组FOB检查与纠正DLTs位置的时间为(38.04±5.36)s,而Ⅱ组为(60.65±9.25)s,两组差有统计学意义(P<0.05)。I组所有23例患者DLTs位置为理想位置,无错位和严重错位,而Ⅱ组理想位置仅有11例,虽然也无严重错位,但有12例错位,两组差异有统计学意义(P<0.05)。结论 支气管套囊充气法定位右侧DLTs具有快速、定位准确等优点,显著提高右侧DLTs盲探插管定位的成功率。

      Objective To investigate the success rate of application of endobronchial cuff inflation method in blind placement of the right-sided double-lumen tubes. Methods Forty-six adult patients were enrolled in this study, who were from the Thoracic Surgery Department of the First Affiliated Hospital of Anhui Medical University, undergoing elective thoracic surgery requiring a right-sided DLT through the period from June 2016 to Dec2017. The patients were randomly allocated to group I or group Ⅱ with 23 patients in each group. In group I, the tubes were located by the method of endobronchial cuff inflation, while in group Ⅱ, the tubes by auscultation. The variables recorded in this study were:the placement of the DLTs, the time for the intubation of DLTs and the time for FOB verification. Results 23(100.00%) of the 23 intubations in group I was considered as an optimal position while only 11(47.83%) of the 23 intubations in group Ⅱ was in the optimal position, and the difference was statistically significant(P<0.05). Time for the intubation of DLTs was mean(SD)(103.22±18.34)s in group I and(99.39±16.31)s in group Ⅱ, whereas no differencewas noted between the two groups(P>0.05); the time for FOB verification was(38.04±5.36)s in group I,which was less than that in group Ⅱ(60.65±9.25) s(P<0.05). Conclusion In conclusion, with the method of endobronchial cuff inflation, the right sided DLTs can be positioned more accurately and more rapidly.
