      目的 探讨音乐训练干预对阿尔茨海默病患者功能性磁共振成像(fMRI)检查结果及汉密尔顿抑郁量表(HAMD)评分的影响。方法 选择2016年6月至2017年7月解放军第一五三中心医院收治的82例阿尔茨海默症患者,以随机数字表法分为观察组和对照组,每组各41例。对照组患者进行常规干预,观察组患者进行音乐训练干预,均干预治疗3个月,比较两组患者干预后汉密尔顿焦虑量表(HAMA)、HAMD、易精神状态量表(MMSE)、活动能力(ADL)、痴呆评定量表(DRS)评分,记录并比较两组患者各量表评分和静息态功能磁共振技术(RS-fMRI)数据局部功能连接密度(LFCD)。结果 干预后观察组的HAMA、HAMD及DRS各项评分均低于对照组,MMSE评分及ADL评分高于对照组,观察组干预后HAMA、HAMD评分降低程度,DRS、MMSE及ADL评分提高程度均高于对照组,差异有统计学意义(P<0.05);对照组患者治疗前后LFCD无明显变化,观察组患者治疗后LFCD较高的区域位于左侧楔前叶,显示该区域脑部活动增强。结论 音乐干预能有效改善阿尔茨海默病患者抑郁及焦虑情况,fMRI检查结果中显示其脑部活动增强,值得临床推广。

      Objective To explore the effects of music training intervention on functional magnetic resonance imaging (fMRI) examination results of patients with alzheimer's disease and HAMA and HAMD scores. Methods 82 patients with alzheimer's disease who were admitted to the 153 Central Hospital of the People's Liberation Army from June 2016 to July 2017 were randomly divided into observation group and control group, with 41 patients in each. The patients in the control group were given routine care, and the patients in the observation group was given music training intervention nursing, with 3 months of intervention. The HAMA, HAMD, MMSE, ADL, DRS score of the two groups after intervention were compared. The scores of the two groups of patients with the scale and resting state functional magnetic resonance (RS-fMRI) data local functional connection density (LFCD) were recorded and compared. Results The scores of HAMA, HAMD and DRS in the observation group were significantly lower than those in the control group, while the scores of MMSE and ADL was significantly higher than that of the control group,The difference in the observation group before and after treatment was significantly higher than that in the control group. (P<0.05). There was no significant change in LFCD before and after treatment in the control group. The area with higher LFCD after treatment in the observation group was located in the left anterior wedge, indicating that the brain activity in this area was enhanced.Conclusion The results of fMRI in patients with Alzheimer's disease after music intervention show that the brain activity is increased and can effectively improve the depression and anxiety in patients with Alzheimer's disease. It is an effective intervention method and worthy of promotion.
