      目的 探讨乳腺钼靶摄影中乳腺癌阳性表现与雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER-2)表达的相关性。方法 收集合肥市第三人民医院2013年8月至2018年10月行乳腺钼靶检查并可见阳性征象的30例乳腺癌患者的临床资料进行回顾性分析,根据其影像学表现分为簇状钙化组、肿块组以及肿块合并簇状钙化组3组。分析影像学表现与组织学来源,影像学表现与免疫组化结果中ER、PR、HER-2的关系。结果 影像学表现为单纯簇状钙化者10例(33.3%),单纯肿块者7例(23.3%),肿块合并簇状钙化者13例(43.3%)。ER、PR、HER-2的阳性率分别为70.0%(21/30)、60.0%(18/30)和56.7%(17/30)。来源于导管上皮的乳腺癌钙化率高于非导管上皮肿瘤(P<0.05)。肿块合并簇状钙化组患者中HER-2表达高于其他两组(P<0.05),且肿块边缘毛刺征象和HER-2表达呈负相关(r=-0.265,P<0.05)。结论 簇状钙化、肿块以及肿块合并簇状钙化是乳腺癌钼靶检查中最重要的三种影像学表现,来源于导管上皮的乳腺癌钙化率较高。HER-2在肿块合并簇状钙化患者中表达最高,且和肿块影像学表现有关。

      Objective To explore the correlation of breast cancer positive performance in mammography and the expression of estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER-2). Methods Retrospective analysis was performed on 30 patients with positive signs of mammography in the Third People's Hospital of Hefei from August 2013 to March 2018. The patients were divided into clustered calcification group, mass group, and mass with clustered calcification group according to their imaging findings. The relationship of imaging features and histological sources and ER, PR and HER-2 in the results of immunohistochemistry were analyzed. Results The imaging findings were simple cluster calcifications in 10 cases (33.3%), simple masses in 7 cases (23.3%), and tumor mass with cluster calcification in 13 cases (43.3%). The positive rate of ER, PR and HER-2 was 70.0% (21/30), 60.0% (18/30) and 56.7% (17/30), respectively. The cluster calcification rate of breast cancer derived from ductal epithelium was significantly higher than that of non-ductal epithelial tumors (P<0.05). The expression of HER-2 in the tumor mass with cluster calcification group was significantly higher than that in the other two groups (P<0.05), and the peripheral burr sign of the tumor was negatively correlated with Her-2 expression (r=-0.265, P<0.05). Conclusion Cluster calcification, mass and mass with cluster calcification are the most important three imaging manifestations in mammography of breast cancer. The calcification rate of breast cancer originating from ductal epithelium is higher than the others. HER-2 expression is the highest in patients with mass and cluster calcification, which is related to the imaging manifestations of mass.
