Objective To investigate the clinical effect of fetal capsular injection of methotrexate combined with complete curettage of uterine cavity under B-mode ultrasound monitoring in the treatment of patients with cesarean scar pregnancy. Methods The clinical data of 87 patients with cesarean scar pregnancy treated in Angang General Hospital from October 2014 to October 2017 were retrospectively analyzed. They were divided into control group (n=43) and experimental group (n=44) according to different treatment regimens. The control group was treated with fetal capsular injection of methotrexate alone, and the experimental group with fetal capsular injection of methotrexate combined with complete curettage of uterine cavity. The clinical effects, menstrual recovery time, the disappearance time of the mass, time of blood human chorionic gonadotropin (HCG) turning negative, vaginal bleeding time, hospitalization time and quality of life score (SF-36) before and after two months of treatment were compared between the two groups. Results After treatment, the total effective rate of the experimental group was 88.64%, higher than that of control group (67.44%), and the difference was statistically significant (P<0.05). Compared with control group, the disappearance time of the mass, the time of HCG turning negative, the time of vaginal bleeding and the time of hospitalization in experimental group were all shorter, and the difference was statistically significant (P<0.05). The SF-36 scores of two groups increased after two months of treatment, and those of experimental group was higher than those of control group with statistically significant difference (P<0.05). Conclusion Fetal capsular injection of methotrexate combined with complete curettage of uterine cavity under B-mode ultrasound monitoring in treatment of patients with cesarean scar pregnancy can obviously improve the clinical effect, shorten the recovery time of patients and improve the quality of life of patients. |
