      目的 探究3D打印数字技术辅助经皮穿刺椎体成形术(PVP)治疗重度骨质疏松性椎体压缩骨折(OVCF)患者的临床效果及安全性。方法 回顾性分析2015年6月至2017年5月深圳市龙岗区第二人民医院收治的52例重度OVCF患者的临床资料,根据手术方法分为观察组(25例)和对照组(27例),观察组术前进行骨折椎体的三维重建,并3D打印椎体模型进行预操作;对照组行常规PVP术。观察并记录两组患者获得最佳穿刺位置时穿刺定位时间、透视次数;于术前、术后第1天、术后12个月时进行视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)检查,测定椎体高度、骨折椎体前缘压缩比及Cobb角,统计并发症发生率。结果 观察组双凹型、扁平型骨折类型及总体穿刺时间、透视次数低于对照组,差异有统计学意义(P < 0.05),术后第1天及术后12个月时,两组患者骨折椎体前缘压缩比、Cobb角、VAS评分、ODI较术前均下降,椎体高度升高,差异均有统计学意义(P < 0.05);两组患者术后12个月时VAS评分低于术后第1天,差异有统计学意义(P < 0.05);观察组骨水泥渗漏发生率低于对照组,差异有统计学意义(P < 0.05)。结论 3D打印辅助PVP术治疗重度OVCF疗效确切,可提高PVP安全性。

      Objective To investigate the effect and safety of percutaneous vertebroplasty (PVP) assisted by 3D printing digital technique in the treatment of patients with severe osteoporotic vertebral compression fracture (OVCF). Methods The clinical data of 52 patients with severe OVCF admitted to Longgang Second People's Hospital, Shenzhen, from June 2015 to May 2017 were retrospectively analyzed. Grouping was performed according to surgical methods. 27 patients undergoing routine PVP were included into control group. 25 patients undergoing three-dimensional reconstruction of the fractured vertebra and vertebral model before surgery and pre-operation were included into observation group. The puncture positioning time when obtaining the best puncture position and the number of fluoroscopy in both group were observed and recorded. The scoring of visual analogue scale (VAS) and Oswestry disability index (ODI) was performed before surgery and at 1 day, 12 months after surgery. The vertebral height, compression ratio of anterior margin of fractured vertebra and Cobb angle were measured. The incidence of complications was statistically analyzed. Results The puncture time in terms of double concave fracture and the flat fracture, and number of fluoroscopy in observation group were lower than those in control group (P < 0.05). The compression ratio of anterior margin of fractured vertebra, Cobb angle, VAS score and ODI score of the two groups at 1d and 12 months after surgery were decreased than those before surgery (P < 0.05), while vertebral height was increased (P < 0.05) The VAS scores at 12 months after surgery in both groups were lower than those at 1 day after surgery. The incidence of bone cement leakage in observation group was lower than that of control group (P < 0.05). Conclusion The curative effect of PVP assisted by 3D printing is significant on severe OVCF, which can improve the safety of PVP.
