      目的 探讨3D打印预处理对A2-3/C2-3型股骨远端骨折患者的疗效及安全性。方法 回顾性分析溧阳市人民医院2015年1月至2018年10月收治的124例A2-3/C2-3型股骨远端骨折患者的临床资料,其中67例采用常规内固定手术治疗设为对照组,57例则在内固定术前采用3D打印预处理设为观察组;比较两组患者围术期指标、随访美国特种外科医院(HSS)评分优良率及术后并发症发生率。结果 观察组手术时间、术中出血量、术中透视次数、术后引流量及住院时间分别为(78.19±23.72)min、(154.94±46.85)mL、(2.82±0.64)次、(52.85±10.34)mL、(12.70±1.64)d,均短/少于对照组,差异有统计学意义(P<0.05);两组随访HSS评分优良率比较,差异无统计学意义(χ2=2.141,P=0.325);观察组术后并发症发生率为10.53%,低于对照组的31.34%,差异有统计学意义(P<0.05)。结论 3D打印预处理辅助用于A2-3/C2-3型股骨远端骨折患者围手术期效果较好、并发症发生率低,安全性尚可。

      Objective To investigate the influence of 3D printing pretreatment on clinical efficacy and safety of patients with distal femoral fractures for A2-3/C2-3. Methods The clinical data of 124 patients with distal femoral fractures for A2-3/C2-3 were retrospectively chosen in People’s Hospital of Liyang City from January 2015 to October 2018 and divided into two groups, namely control group (67 patients) with conventional internal fixation operation and observation group (57 patients) with 3D printing pretreatment on the basis of control group; and the intraoperative and postoperative clinical indicators, excellent and good rate of HSS score and postoperative complications incidence of the two groups were compared. Results The operation time, intraoperative bleeding volume, intraoperative fluoroscopy times, postoperative drainage and hospital staying time of observation group were significantly less than those of control group (P<0.05). There was no significant difference in reduction satisfaction rate, fracture healing time and excellent and good rate of HSS score between two groups (P>0.05). The incidence of postoperative complications of observation group were significantly lower than that of control group (P<0.05). Conclusions Compared with conventional internal fixation operation used alone, 3D printing pretreatment in the treatment of patients with distal femoral fractures for A2-3/C2-3 possesses the same functional recovery effects but can effectively shorten the operation time, decrease iatrogenic trauma, reduce radiation hazards, speed up the process of rehabilitation after operation and be helpful to prevent the incidence of postoperative complications.
