      目的 探讨碘染粉色征与窄带成像背景着色征对早期食管癌筛查的诊断价值。方法 回顾性分析2016年11月至2019年9月在安徽医科大学附属安庆医院住院疑诊早期食管癌43例(共计47处病变)患者临床资料,以病理诊断作为金标准,对比分析内镜下碘染粉色征与窄带成像背景着色征诊断早期食管癌的准确率、灵敏度、特异度。结果 碘染粉色征诊断早期食管癌的诊断准确率为87.23%(41/47)、灵敏度为91.67%(22/24)、特异度为82.61%(19/23)。窄带成像背景着色征诊断早期食管癌的诊断准确率为89.36%(42/47)、灵敏度为87.50%(21/24)、特异度为91.30%(21/23)。两种方法诊断准确率差异无统计学意义(χ2=0.571,P>0.05)。结论 碘染色和窄带成像对早期食管癌均具有较高的诊断准确率和灵敏度,但碘染色操作费时且有不良反应,而窄带成像操作简便并可反复观察,值得临床推广使用。

      Objective To investigate the diagnostic value of iodine staining pink color sign(PCS) and narrow-band imaging background coloration sign (BCS) for early esophageal cancer screening. Methods A retrospective analysis wasperformed on clinical data of 43 patients(a total of 47 lesions), who were admitted to underwent endoscopy with iodine staining and narrow-band imaging to screen early diagnosis of esophageal cancer in Anqing Hospital Affiliated to Anhui Medical University from November 2016 to September 2019. The pathological diagnosis was determined as the gold standard for all lesions. The diagnostic accuracy, sensitivity and specificity of iodine staining PCS and narrow-band imaging BCS for the early esophageal cancer were compared. Results The diagnostic accuracy, sensitivity and specificity of iodine staining PCS in the diagnosis of early esophageal cancerwas 87.23% (41/47), 91.67% (22/24) and 82.61% (19/23), respectively, and the corresponding statistical value of narrow-band imaging BCS was 89.36% (42/47), 87.50% (21/24) and 91.30% (21/23), respectively. The difference in diagnostic accuracy between the two methods was not statistically significant (χ2=0.571, P>0.05). Conclusions Both iodine staining PCS and narrow-band imagingBCS have high accuracy and sensitivity for early esophageal cancer screening. Iodine staining is time-consuming and has adverse reactions, while narrow-band imagingis easy to operate and can be observed repeatedly, which is worthclinical application.
