目的 探讨肥胖患者脂肪组织神经酰胺(Cer)水平与并发糖尿病的相关性。方法 选取2018年1月至2020年10月在安徽医科大学第二附属医院胃肠外科进行减重手术的200例肥胖患者为研究对象,分为肥胖并发糖尿病(OD)组(n=56)与肥胖非糖尿病(OND)组(n=144)。采集两组患者静脉血、手术切除的腹腔脂肪组织及临床资料。使用液相色谱-串联质谱法(LC-MS/MS)检测脂肪组织中Cer浓度,酶联免疫吸附试验检测血清和脂肪组织炎性细胞因子白介素(IL)-1β和IL-18水平。秩和检验比较两组患者脂肪组织Cer浓度、炎性细胞因子水平差异;Spearman相关分析脂肪组织中Cer浓度与炎性细胞因子水平的相关性;logistic回归分析探讨脂肪组织Cer水平与肥胖并发糖尿病的关系。结果 OD组患者脂肪组织C16-Cer、C18-Cer、C24-Cer、C24:1-Cer及总Cer均高于OND组(P<0.05);OD组患者血清和脂肪组织IL-1β、IL-18水平均高于OND组(P<0.05);脂肪组织Cer浓度与IL-1β、IL-18水平呈正相关(P<0.05);logistic回归分析显示,脂肪组织Cer是肥胖并发糖尿病的危险因素(P<0.05)。结论 肥胖患者脂肪组织Cer水平与脂肪组织炎症相关,增加并发糖尿病的风险。 |

Objective To investigate the correlation between adipose ceramide (Cer) levels and diabetes mellitus in obese patients. Methods Two hundred obese patients undergoing weight loss surgery were selected as the research objects, and divided into obese with diabetes group (OD) and obese non-diabetic group (OND). Venous blood, abdominal adipose tissue and clinical data were collected. The concentration of Cers in adipose tissues was determined by liquid chromatography-tandem mass spectrometry (LC-MS/MS), and the levels of inflammatory cytokines interleukin (IL) -1β and IL-18 in serum and adipose tissue were determined by enzyme-linked immunosorbent assay (ELISA). The Cers concentrations and inflammatory cytokine levelswere compared between OD group and OND group by rank sum test. Spearman correlation analysis was performed to determine the correlation between Cers concentration and inflammatory cytokine levels in adipose tissues. Logistic regression was used to investigate the relationship between Cer levels in adipose tissues and obesity complicated with diabetes. Results C16-Cer, C18-Cer, C24-Cer, C24:1-Cer and total Cers in OD group were higher than those in OND group(P<0.05). The levels of IL-1β and IL-18 in serum and adipose tissues of OD group were higher than those of OND group(P<0.05). The Cer concentration in adipose tissues was positively correlated with the levels of IL-1β and IL-18(P<0.05). Logistic regression showed that adipose tissue Cer was a risk factor for obesity complicated with diabetes(P<0.05). Conclusions Ceramides in adipose tissues are associated with adipose tissue inflammation, which may increase the risk of diabetes in obese patients. |
