目的 建立伏立康唑药物基因检测与血药浓度监测方法,指导伏立康唑临床个体化用药。方法 选取2020年6月至2021年6月在安徽医科大学附属巢湖医院血液内科接受伏立康唑治疗或预防侵袭性真菌感染的10例血液病患者为研究对象,采用荧光原位杂交技术检测CYP2C19基因多态性;并使用酶联免疫放大法测定伏立康唑血药浓度,根据基因型和血药浓度监测结果制定或者调整伏立康唑临床治疗方案。结果 10例患者的CYP2C19基因多态性检测结果为快代谢型3例、中间代谢型5例和慢代谢型2例;酶联免疫放大法测定伏立康唑血药浓度,该方法精密度、稳定性和回收率均符合相关要求。根据基因型调整伏立康唑剂量后,患者血药浓度均在推荐范围内(0.5~5.0 μg/mL)。结论 CYP2C19基因检测联合血药浓度监测,可以指导伏立康唑临床个体化用药。 |

Objective To establish genetype detection and blood concentration monitoring method of voriconazole for guiding clinical individualized medication. Methods Ten patients with hematological diseases who received voriconazole treatment or prevention of invasive fungal infections in the Chaohu Hospital of Anhui Medical University were selected as the research objects. Fluorescence in situ hybridization was used to detect CYP2C19 gene polymorphism; and enzyme-linked immunoamplification method was used to determine the plasma concentration of voriconazole. The clinical treatment plan of voriconazole was developed or adjusted based on the results of genotype and blood concentration monitoring. Results The results of the CYP2C19 gene polymorphism test of 10 patients were 3 cases of fast metabolizer, 5 cases of intermediate metabolizer and 2 cases of slow metabolizer. The plasma concentration of voriconazole was determined by enzyme-linked immunoamplification method. The precision, stability and recovery rate of the method met the relevant requirements. After adjusting the dose of voriconazole according to the genotype, the patient plasma concentration was within the effective range (0.5~5 μg/mL). Conclusion Genetype detection and blood concentration monitoring can guide the clinical individualized use of voriconazole. |
