| 目的 探讨肺部超声在成人社区获得性肺炎(CAP)诊断中的临床应用价值。方法 选择2019年3月至2020年9月在安庆市立医院首诊疑似CAP的89例住院患者,收集肺部超声及胸部CT检查资料,将二者结果与临床确诊结果进行对照,对二者诊断CAP的临床价值进行评价,并总结肺炎患者超声声像特征。结果 89例患者中,临床确诊肺炎84例,超声诊断肺炎83例,敏感度为97.62%,特异度为80.00%,准确度为96.63%,阳性预测值为98.80%,阴性预测值为66.67%;CT诊断肺炎85例,敏感度为98.81%,特异度为60.00%,准确度为96.63%,阳性预测值为97.65%,阴性预测值为96.63%。一致性检验结果显示,超声诊断肺炎与临床确诊肺炎有较高的一致性(Kappa=0.709,P<0.01);CT诊断肺炎与临床确诊肺炎亦有较高一致性(Kappa=0.649,P<0.001)。结论 肺部超声诊断成人CAP与临床诊断具有较高一致性,与胸部CT有同等的敏感度、准确度,临床应用价值较高,值得推广。 |

| Objective To discuss the clinical value of pulmonary ultrasound in the diagnosis of community-acquired pneumonia (CAP)in adults. Methods A total of 89 patients with suspected CAP who were first diagnosed in Anqing Municipal Hospital from March 2019 to September 2020 were selected, and the examination data of lung ultrasound and chest computed tomography(CT)were collected, then their diagnosis results were compared with the clinical diagnosis, the clinical value of lung ultrasound and chest CT in the diagnosis of CAP was evaluated, and the ultrasonographic features of patients with pneumonia were summarized. Results In all 89 patients, 84 cases were clinically confirmed with pneumonia; 83 cases were diagnosed by lung ultrasound,and the sensitivity, specificity, accuracy, positive predictive value and negative predictive was 97.62%, 80.00%, 96.63%, 98.80% and 66.67% respectively;83 cases were diagnosed by chest CT, the sensitivity, specificity, accuracy, positive predictive value and negative predictive was 98.81%, 60.00%, 96.63%, 97.65% and 96.63% respectively. The results of consistency test showed that there was a high consistency between clinical diagnosis with lung ultrasound (Kappa=0.649,P<0.001) and CT diagnosis(Kappa=0.649,P<0.001). Conclusions Lung ultrasound has good consistency with clinical diagnosis,and has the same sensitivity and accuracy as chest CT in the diagnosis of CAP in adults.It has high clinical application value and is worth popularizing. |
