      目的 探讨老年冠心病患者的衰弱评估与6 min步行试验(6MWT)的相关性。方法 选取2019年8月至2020年1月在池州市人民医院心内科住院行冠脉造影检查确诊为冠心病的老年患者60例(年龄 ≥ 60岁),且均行6MWT检查,根据加拿大健康与衰老研究中心的临床衰弱评估量表(CFS),将CFS评分 ≥ 5分的纳入临床衰弱组(20例),将评分CFS评分<5分的纳入对照组(40例),分析老年冠心病衰弱患者的危险因素,比较衰弱组和对照组随访1年因心血管事件再住院率,评估6MWT对老年冠心病衰弱患者的临床应用价值。结果 衰弱组的6MWD为(294.75±96.67)m,对照组为(362.10±80.61)m,两组差异有统计学意义(P=0.006),且衰弱组的心血管事件再次住院率明显高于对照组,差异有统计学意义(P=0.019)。老年冠心病患者评估为衰弱的多因素二分类logistic回归分析的结果显示6MWD是老年冠心病患者评估为衰弱的独立危险因素(P=0.014)。结论 老年冠心病合并衰弱的患者具有更高的心血管事件再住院率,6MWD是老年冠心病患者评估为衰弱的危险因素,6MWT可用于老年冠心病衰弱患者的综合评估。

      Objective To investigate the relationship between frailty assessment and six-minute walk test in elderly patients with coronary heart disease. Methods Sixty elderly patients (≥ 60 years old) with coronary heart disease confirmed by coronary angiography were selected, who were admitted to the Department of Cardiology of Chizhou People's Hospital from August 2019 to January 2020. All patients underwent 6MWT examination. According to the Clinical Frailty Scale (CFS) from Canadian Research Centre for Health and Aging, those with CFS score greater than or equal to 5 were included in the clinical frailty group (20 cases), and those with CFS score less than 5 were included in the control group (40 cases), and the risk factors of the elderly patients of frailty with coronary heart disease were analyzed. The readmission rates due to cardiovascular events were compared between the two groups during 1-year follow-up, and the clinical predictive value of 6MWT for the elderly patients of frailty with coronary heart disease was evaluated. Results The 6min walking distance of the frailty group (6MWD) was (294.75±96.67) m, and that of the control group 6MWD was (362.10±80.61) m. The difference between the two groups was statistically significant (P=0.006), and the rehospitalization rate of cardiovascular events in the fattening group was significantly higher than that in the control group (P=0.019). Multivariate binary logistic regression analysis showed that 6MWD was an independent risk factor for frailty in elderly patients with coronary heart disease (P=0.014). Conclusions Elderly patients with coronary heart disease combined with frailty have a higher rate of rehospitalization for cardiovascular events. 6MWD is an independent risk factor for frailty in elderly patients with coronary heart disease, and 6MWT can be used for comprehensive evaluation of elderly patients with frailty of coronary heart disease.
