| 目的 探讨人工耳蜗植入患儿父母悦纳进取水平,分析亲职压力、家庭坚韧力和悦纳进取之间的路径关系。方法 2018年5月至2020年4月以横断面调查的方式便利选取南京医科大学附属儿童医院行单侧耳蜗植入的295例重度或极重度神经性耳聋患儿父母为研究对象,采用一般资料调查表、亲职压力量表、家庭坚韧力量表(FHI)、悦纳进取量表进行调查,使用 Pearson 相关分析亲职压力、家庭坚韧力和悦纳进取的相关性;采用Amos 21.0软件建立结构方程模型,分析家庭坚韧力在人工耳蜗植入患儿父母亲职压力与悦纳进取水平间的中介作用。结果 患儿父母悦纳进取量表总分为(75.64±12.56)分,亲职压力得分为(81.33±20.92)分,家庭坚韧力量表得分为(58.86±12.41)分。Pearson相关分析结果显示,亲职压力和家庭坚韧力呈负相关(r=-0.286,P<0.05),和悦纳进取呈负相关(r=-0.293,P<0.05),家庭坚韧力和悦纳进取呈正相关(r=0.304,P<0.05)。结构方程模型结果显示,人工耳蜗植入患儿父母亲职压力对悦纳进取有直接(负向)效应(β=-0.374,P<0.05),亲职压力还可通过家庭坚韧力对悦纳进取产生影响,家庭坚韧力在亲职压力和悦纳进取间存在中介效应(β=-0.142,P<0.05)。结论 家庭坚韧力在亲职压力和悦纳进取之间起中介作用,医护人员可从减轻亲职压力、促进家庭坚韧力入手,帮助患儿父母提高悦纳进取水平。 |

| Objective To explore the acceptance and enterprising level of parents of children with cochlear implants, and to analyze the path relationship between parental pressure, family tenacity and positive acceptance. Methods From May 2018 to April 2020, 295 children with severe or severe neurological deafness who underwent unilateral cochlear implantation in a tertiary A general hospital were selected as the research objects by cross-sectional survey. General information questionnaire, parental stress scale, family resilience scale (FHI), acceptance and enterprising scale were used to conduct surveys. Pearson correlation analysis was performed to analyze parental pressure, family resilience and acceptance and enterprising. Amos 21.0 software was used to establish a structural equation model to analyze the mediating role of family resilience between parental stress and acceptance enterprising level in children with cochlear implantation.Results The parental acceptance score of the children was (75.64±12.56) points, the parental stress score was (81.33±20.92) points, and the family resilience scale score was (58.86±12.41) points. The results of Pearson correlation analysis showed, parental pressure was negatively correlated with family tenacity (r=-0.286,P<0.05), negatively correlated with acceptance and enterprising (r=-0.293,P<0.05), family tenacity was positively correlated with accepting enterprising (r=0.304,P<0.05); structural equation model showed that direct (negative) effect of parental pressure on acceptance and enterprising(β=- 0.37,P<0.05), parental pressure could also affect Yuena enterprise through family tenacity, and family tenacity had an intermediary effect between parental pressure and acceptance and enterprising (β=- 0.14,P<0.05).Conclusions Family resilience plays an intermediary role between parental pressure and acceptance and enterprising. Medical staff can start by reducing parental pressure and promoting family resilience to help parents of children with children improve their acceptance and enterprising level. |
